New tool assesses patients' home dialysis experience

New tool assesses patients' home dialysis experience | Peritoneal dialysis | Scoop.it

Home dialysis, which includes both peritoneal dialysis and home hemodialysis, allows patients to receive their dialysis treatments at home, gives patients independence and flexibility with their treatment schedules, and is associated with similar or better outcomes and lower costs compared with conventional in-center hemodialysis. Home dialysis modalities are the fastest growing types of dialysis treatments in the United States, and this trend is likely to continue as recent policies provide incentives to dialysis providers and kidney specialists to promote home dialysis.

Seventy-six of the nephrology trainees completed the survey. The researchers found that regarding the principles of PD, most respondents were moderately confident. However, only 3 and 11 percent had initiated patients on urgent-start PD and observed a PD catheter insertion, respectively. For HHD, the level of confidence was low. Most trainees attended a home dialysis continuity clinic with faculty mentorship.

Peritoneal Dialysis Basics and Benefits

Peritoneal Dialysis Basics and Benefits | Peritoneal dialysis | Scoop.it

PD differs from other dialysis options in that it uses the body’s own abdominal lining (the peritoneum) to clean the blood rather than a synthetic filter (dialyzer). If you choose PD, you’ll need a PD catheter surgically placed, typically through your abdomen. Typically the catheter exit site is allowed to heal for a few weeks, before you begin PD treatments.

COVID-19 in Peritoneal Dialysis Patients

COVID-19 in Peritoneal Dialysis Patients | Peritoneal dialysis | Scoop.it

According to our findings, the incidence of symptomatic COVID-19 in patients on PD was close to that of the general population in the same city (3), indicating that the PD population was not a high-risk population for COVID-19. The multiple and severe comorbidities, but not COVID-19 itself, may contribute to the prolonged hospitalization and mortality of patients on PD. Most of the infected patients successfully developed antibodies against SARS-CoV-2 and recovered from COVID-19 with maintained PD treatment. The characteristics of home-based treatment of PD and the timely epidemic intervention measures helped to minimize the spread of the infection. Considering that the epidemic may enter into regular circulation (5), these findings provide references for the clinical management of the PD population in the COVID-19 outbreak.

The efficacy of peritoneal dialysis (PD) to treat acute kidney injury (AKI) was brought to light after a randomized control trial demonstrated PD is not inferior to intermittent hemodialysis (iHD) for treating acutely ill patients with AKI stage 3 in terms of mortality. Additionally, its ability to be used with minimal infrastructure in place, including the lack of need for water or electricity, made it a novel therapy in many third-world countries.

Peritoneal Dialysis in the United States: Lessons for the Future

Peritoneal Dialysis in the United States: Lessons for the Future | Peritoneal dialysis | Scoop.it

In Hong Kong where there is a PD first policy, 71% of patients use PD, while in Mexico 61 % of patients use PD largely due to lack of availability of other forms of dialysis. In countries with robust education programs (Australia, New Zealand, Canada) approximately 20-30 % of patients use PD. In contrast, among maintenance dialysis patients in the United States (US), the prevalence of PD reached a low of 6.9% in 2009, although the prevalence increased to 10.1% in 2017. Additionally, the proportion of incident dialysis patients initiating dialysis with PD over the same time period has increased from 6.2% to 10.4%. This in part is due to the bundled payment system introduced in 2011. Critically, although the number of patients treated with PD is increasing in the US, it remains lower than what many nephrologists perceive it should be.

Absence of SARS-CoV-2 in the effluent of peritoneal dialysis patients

Absence of SARS-CoV-2 in the effluent of peritoneal dialysis patients | Peritoneal dialysis | Scoop.it

Nasopharyngeal swabs obtained at admission showed high viral load in all three patients, whereas none of the PD effluent specimen tested positive, even after dialysate concentration. Those results support at most a very low SARS-CoV-2 dissemination risk by the peritoneal effluent of PD patients. Imposing special disposal procedures, such as the instillation of hypochlorite in the drainage bags to prevent viral spread to health-care workers, are probably not required.

Increasing Peritoneal Dialysis Use in Response to the COVID-19 Pandemic: Will It Go Viral?

Increasing Peritoneal Dialysis Use in Response to the COVID-19 Pandemic: Will It Go Viral? | Peritoneal dialysis | Scoop.it

As the novel coronavirus disease 2019 (COVID-19) global pandemic has unfolded, efforts to facilitate social distancing while minimizing person-to-person transmission of severe acute respiratory syndrome coronavirus 2 has resulted in unprecedented changes across all facets of society. The COVID-19 pandemic represents an opportunity to reevaluate and refine existing models of health care delivery to enable provision of high-quality, comprehensive care traditionally offered within health care facilities in the home. In the United States and most high-income countries, dialysis is predominantly delivered as in-center hemodialysis, although the minority of patients who undergo dialysis at home, either as home hemodialysis or peritoneal dialysis (PD), have comparable, if not better, outcomes.

While the Executive Order set forth bold goals to increase home dialysis utilization amongst patients with end stage renal disease (ESRD),1 the COVID-19 pandemic has served as an even greater impetus to allow appropriate patients to dialyze at home. In many areas, the cancellation of elective surgeries has led to delays in getting appropriate accesses created for our ESRD patients. For some, timely access creation was an issue even before the pandemic.
