Not only adequate uremic toxin removal but also volume control is essential in peritoneal dialysis (PD) to improve patient outcome. Modification of dwell time impacts on both ultrafiltration (UF) and purification. A short dwell favors UF but preferentially removes small solutes such as urea. A long dwell favors uremic toxin removal but also peritoneal fluid reabsorption due to the time-dependent loss of the crystalloid osmotic gradient.

People with advanced kidney disease can be treated with peritoneal dialysis (PD) which involves a catheter being permanently inserted into the lining around the abdominal organs (the peritoneum) through the abdominal wall. Sterile fluid is drained in and out several times each day. The peritoneal lining enables movement of salts and toxins that accumulate when kidney function cannot maintain usual function. Wastes from the bloodstream are moved into the dialysis fluid and removed with the fluid as it is drained from the body.

A population-based approach indicates an overall higher patient mortality with peritoneal dialysis compared to hemodialysis in Korea

To date, only a few large-scale studies have measured the effect of dialysis modality on mortality in Asian populations. Here, we sought to compare survival between incident hemodialysis (HD) and peritoneal dialysis (PD) patients using the Korean Health Insurance Review & Assessment Service database. This enabled us to perform a population-based complete survey that included 32,280 incident dialysis patients and followed them for a median of 26.5 months.

The impact of dialysis modality survival is still somewhat controversial because there are conflicting research results about the survival differences between Peritoneal Dialysis (PD) and Hemodialysis (HD), especially during the first 2 years of dialysis treatment. The survival discrepancies among Chronic Kidney Disease (CKD) patients using different dialysis modalities is hard to pin-down because of differential rates of modality switching, varying rates of Kidney Transplantation, and possible differences in patients’ personal characteristics leading to death.

I met a 26 year old woman recently who was referred by a friend of the family for a second opinion. I seem to get a lot of new patients in this way.  She was found to have advanced kidney failure, and needed to go on dialysis immediately.  Unfortunately, the prior year she had seen her primary care doctor for headaches and was noted to have a blood pressure of 140/85.  But no kidney function tests were given.

As the incidence of end stage renal disease increases across the globe, so too do the survival rates of peritoneal dialysis patients. It is notable though, that peritoneal dialysis utilization does not follow at the same pace, attributable into the high technique failure rates, mainly due to peritoneal catheter dysfunction. A new systematic review and meta-analysis by Hagen et al. reveals that the use of straight catheters may improve outcomes and technique survival.

398,861 people with End Stage Renal Disease (ESRD) are being treated with some form of dialysis while 172,553 ESRD patients have a working Transplanted Kidney, according to the  National Institutes of Health (NIH).  Of those patients, only 8% conduct dialysis at home, and yet an even smaller percentage of people who have ESRD conduct Peritoneal Dialysis (PD). The National Kidney and Urologic Diseases Information Clearinghouse (NKUDIC), notes that 26,114 dialysis patients use PD, 2,455 people choose home Hemodialysis (HD), and the vast majority of people on dialysis conduct in-center HD (370,292).
