Why mechanical thrombectomy is a powerful, yet often underutilized, form of stroke treatment

Why mechanical thrombectomy is a powerful, yet often underutilized, form of stroke treatment | Neurovascular Intervention | Scoop.it

Mechanical thrombectomy is a minimally invasive procedure that offers positive benefits to ischemic stroke patients – which account for 85 percent of all strokes – in the first few hours after stroke onset.

Stroke Severity and Early Ischemic Changes Predict Infarct Growth Rate and Clinical Outcomes in Patients With Large‐Vessel Occlusion

Stroke Severity and Early Ischemic Changes Predict Infarct Growth Rate and Clinical Outcomes in Patients With Large‐Vessel Occlusion | Neurovascular Intervention | Scoop.it

The infarct growth rate (IGR) measures ischemic stroke progression and varies among patients. Clinicoradiological phenotypes of IGR are poorly understood. We evaluated the association of presentation stroke severity and early ischemic changes with infarct progression in patients who underwent successful thrombectomy.

Preventing Recurrent Strokes with Carotid Drug-Eluting PTA for Delayed In-Stent Neointimal Hyperplasia

Preventing Recurrent Strokes with Carotid Drug-Eluting PTA for Delayed In-Stent Neointimal Hyperplasia | Neurovascular Intervention | Scoop.it

In conclusion, Tandem occlusions (Extracranial and Intracranial) represent challenging high-risk scenarios with potential for devastatingly poor outcomes. Combination approaches of mechanical thrombectomy and Carotid PTA and Stent may result in favorable independent outcomes in as many as 1 out of 2 patients if performed early. These patients should be managed cautiously in follow-up in that as many as 3-20% of patients may experience ISR (majority asymptomatic), that may place them at higher risk for Recurrent Stroke. Many of these patients should be strongly considered for early intervention, especially if presenting with very high grade ISR (>70%) or recurrent symptoms.

Mesenchymal Stem Cell Derived Extracellular Vesicles for Repairing the Neurovascular Unit after Ischemic Stroke

Mesenchymal Stem Cell Derived Extracellular Vesicles for Repairing the Neurovascular Unit after Ischemic Stroke | Neurovascular Intervention | Scoop.it

Ischemic stroke is a debilitating disease and one of the leading causes of long-term disability. During the early phase after ischemic stroke, the blood-brain barrier (BBB) exhibits increased permeability and disruption, leading to an influx of immune cells and inflammatory molecules that exacerbate the damage to the brain tissue. Mesenchymal stem cells have been investigated as a promising therapy to improve the recovery after ischemic stroke.

"We have finally conquered the basilar artery” with EVT

"We have finally conquered the basilar artery” with EVT | Neurovascular Intervention | Scoop.it

Nogueira began his presentation by noting that, in spite of the “overwhelming” proof of its benefit in treating anterior circulation large vessel occlusion (LVO) strokes, it remains unknown whether EVT offers the same advantages in acute BAOs—despite “Herculean” efforts seen in the BEST and BASICS trials. He also noted positive signals seen with EVT in these studies, and said they “paved the way” for the ATTENTION study by demonstrating the importance of consecutive recruitment, fast enrolment and minimal crossover, as well as confirming the ideal target population.

Endovascular treatment of wide-neck intracranial bifurcation aneurysms with ‘Y’-configuration, double Neuroform® stents-assisted coiling technique

Endovascular treatment of wide-neck intracranial bifurcation aneurysms with ‘Y’-configuration, double Neuroform® stents-assisted coiling technique | Neurovascular Intervention | Scoop.it

Intracranial wide-neck aneurysms at the arterial bifurcations, especially in the aneurysms where the bifurcating branches emanate directly from the base of the aneurysm, have been particularly difficult on which to perform endovascular treatment. The ‘Y’-configuration, double stent-assisted coil embolization is an option for the treatment of these difficult aneurysms, allowing the closure of the aneurysm, preserving the parent arteries.

Is China Still A Good Opportunity For Device Manufacturers?

Is China Still A Good Opportunity For Device Manufacturers? | Neurovascular Intervention | Scoop.it

The best way to succeed in China is to focus on selling unique products there; unique products that are needed, where the reimbursements are still relatively good, and VBP will not reduce prices, yet can still yield large sales. Western brands are still perceived as higher quality than locally made products, and are popular with Chinese patients as long as the pricing is reasonable. Other strategies to be successful in China include licensing or selling your technology since there is more Chinese capital available these days. Another improved factor, although still not great, includes better intellectual property protection in China. Some Western companies are also making more of their products in China, which can help the company to be closer to their customers, in some cases make registration easier, and may reduce manufacturing costs.

We performed a one-point, lipophilic cell-tracer injection to track endothelial cells, followed by an arteriotomy and suturing of sidewall aneurysms on the abdominal rat aorta. Neointima formation seemed dependent on the parent artery in decellularized aneurysms and was promoted by the recruitment from aneurysm wall cells in vital cell-rich walls.
