These data also suggest that such patients have a substantial potential to retain their prestroke level of disability when treated, despite their generally worse prognosis overall, although this remains to be validated in higher-quality registries and clinical trials. By pairing pragmatic and transparent decision-making in clinical practice with an active pursuit of high-quality research, we can work toward a more inclusive paradigm of patient-centered care for this often-neglected patient population.

Endovascular thrombectomy has developed alongside advances in brain imaging methods that help physicians determine which patients are most likely to benefit from this treatment. NINDS-supported research was essential to understanding stroke injury progression, developing the first device approved for endovascular thrombectomy, applying novel imaging methods to acute stroke evaluation, and defining brain imaging profiles that, together with time since symptom onset, guide treatment decisions for acute stroke. Urgent medical attention remains imperative for stroke, as increasing numbers of neurons die for every minute of blocked blood flow. However, endovascular devices and sophisticated brain imaging enable good outcomes for more stroke patients and at later times after stroke onset than was once thought possible.

The Prospective Registry for Assessment of Acute Ischemic Stroke Patients Treated with Neurothrombectomy Devices in India (PRAAN) is an industry-first endeavour to create a post-market registry to assess clinical outcomes associated with the use of Medtronic market-released revascularization devices in patients experiencing acute ischemic stroke. The duration of the registry will be approximately 28 months and up to 200 subjects will be enrolled in it. Devices that are used to restore blood flow in the brain by mechanically removing blood clots from occluded blood vessels are called neurothrombectomy (or mechanical thrombectomy) devices, which include stent retrievers and aspiration catheters.

Saccular aneurysms (SAs) are focal outpouchings from the lateral wall of an artery. Depending on their morphology and location, minimally invasive treatment options include coil embolization, flow diverter stents, stent-assisted coiling, and liquid embolics. Many drawbacks are associated with these treatment options including recanalization, delayed healing, rebleeding, malpositioning of the embolic or stent, stent stenosis, and even rupture of the SA. To overcome these drawbacks, a nanoclay-based shear-thinning hydrogel (STH) is developed for the endovascular treatment of SAs. Extensive in vitro testing is performed to optimize STH performance, visualization, injectability, and endothelialization in cell culture.

In the actual clinical practice of carotid stenting, there is this risk. 66% of strokes occur after removal of the cerebral protection device because of plaque prolapse through the stent struts. For the single-layer braided process, this risk of embolism due to plaque prolapse is present and high. Based on this risk, double weave stents are used.

Our study shows that high rates of favorable clinical outcome and successful reperfusion can be achieved with EVT for PCS, despite high mortality. Characteristics and outcomes of patients treated in trial versus nontrial centers were similar indicating that our cohort is representative of clinical practice in the Netherlands. Randomized studies using modern treatment approaches are needed for further insight in the benefit of EVT for PCS.

Thrombus imaging characteristics are associated with treatment success and functional outcomes in stroke patients. However, assessing these characteristics based on manual annotations is labor intensive and subject to observer bias. Therefore, we aimed to create an automated pipeline for consistent and fast full thrombus segmentation.

Acupuncture has been used in stroke rehabilitation in China for over 3000 years. However, its mechanisms are still under discussion. Most researchers used biochemical or anatomical tests to observe the animals or electrophysiological tests to observe human beings and tried to explain the mechanisms [1–3]. Unfortunately what they got was just indirect evidence. PET (positron emission tomography), a vivo functional examination technology can be done without invasive procedures. Its image reflects not only the structure of the organs and tissues, but also their physiological and biochemical changes.

A stroke occurs if the flow of oxygen-rich blood to a portion of the brain is blocked. Without oxygen, brain cells start to die after a few minutes. Sudden bleeding in the brain also can cause a stroke if it damages brain cells. If brain cells die or are damaged because of a stroke, symptoms occur in the parts of the body that these brain cells control. Examples of stroke symptoms include sudden weakness; paralysis or numbness of the face, arms, or legs (paralysis is an inability to move); trouble speaking or understanding speech; and trouble seeing.

An ischemic stroke can be caused by thrombosis and ischemia, which is a major public health problem around the world, resulting in severe disability and a high death rate. The goal of this work is to examine and target various heat shock proteins (HSPs) via their interacting partners, which may have an anti-ischemic stroke impact.

Six randomized controlled clinical trials have assessed whether mechanical thrombectomy (MT) alone is non-inferior to intravenous thrombolysis (IVT) plus MT within 4.5 hours of symptom onset in patients with anterior circulation large vessel occlusion (LVO) ischaemic stroke and no contraindication to IVT. An expedited recommendation process was initiated by the European Stroke Organisation (ESO) and conducted with the European Society of Minimally Invasive Neurological Therapy (ESMINT) according to ESO standard operating procedure based on the GRADE system.

More than 25 years ago, the potentially miraculous clinical outcomes that could be achieved after endovascular therapy (EVT) for acute ischemic stroke (AIS) were realized in routine clinical practice. Despite interval progress in establishing evidence, the next chapter in regulatory approval and reimbursement for EVT (EVT 2.0) remains stifled by the failure to advance regulatory science and address the key gaps in the translation of ideal paradigms for AIS treatment into real‐world practice of precision medicine.

A multicenter trial in Japan reported that patients who had experienced an acute stroke with a large infarct size had significantly improved functional outcomes when treated with endovascular therapy (EVT) compared with those receiving medical care alone. The study is the first randomized evidence on the benefit of EVT for patients with large infarct sizes and is one of several randomized trials underway to evaluate the efficacy of EVT in these patients.

The use of a combination of balloon guide catheter (BGC), aspiration catheter, and stent retriever in acute ischemic stroke thrombectomy has not been shown to be better than a stent retriever and BGC alone, but this may be due to a lack of power in these studies. We therefore performed a meta-analysis on this subject.

There is no clear consensus on the optimal endovascular treatment strategy for patients with ischemic stroke caused by ICAS-related large vessel occlusion (LVO). SPACEMAN, a novel thrombectomy technique that entails passing an aspiration catheter over the stent retriever and then retaining the microwire for angioplasty, has not been described. The aim of this prospective study was to evaluate our initial application of SPACEMAN and compare this technique with the Solumbra technique.

Treatment of acute ischaemic stroke (AIS) focuses on rapid recanalisation of the occluded artery. In recent years, advent of mechanical thrombectomy devices and new procedures have accelerated the analysis of thrombi retrieved during the endovascular thrombectomy procedure. Despite ongoing developments and progress in AIS imaging techniques, it is not yet possible to conclude definitively regarding thrombus characteristics that could advise on the probable efficacy of thrombolysis or thrombectomy in advance of treatment. Intraprocedural devices with dignostic capabilities or new clinical imaging approaches are needed for better treatment of AIS patients. In this review, what is known about the composition of the thrombi that cause strokes and the evidence that thrombus composition has an impact on success of acute stroke treatment has been examined.
