One focus of stem cell-based therapy for stroke is using mesenchymal stem cells (MSCs). MSCs are a type of stem cell found in many different tissues in the body and can potentially differentiate into other cell types, including neurons. MSCs have been studied for their potential to treat neurological disorders, including ischemic brain injury, acute ischemic stroke, hemorrhagic stroke, and chronic stroke. They have shown promising results in preclinical and early clinical studies.

Preclinical studies have proven the efficacy of stem cell therapy for ischemic stroke using IA transplantation. Although clinical studies have demonstrated the safety of IA transplantation, efficacy is yet to be established. Multiple confounding variables, including optimal timing, cell dose, cell type, stroke type, stroke severity, and patient age, would explain the discrepancy in treatment efficacy between preclinical and clinical studies. Therefore, optimization of IA stem cell transplantation is necessary, and then further experiments in basic research are needed to fill these discrepancies and confirm the optimal conditions for IA stem cell transplantation.

A 2022 study crunched how much exactly treatment would cost per stroke patient. Short-term it’s around $149,180, with the price to reach $249,780 per stroke patient in the long-term. Luckily, stroke can be prevented and can be treated – if patients get medical help quickly – because the longer treatment is delayed, the more damage the stroke can cause.

“In neurology, we often say that ‘time is brain’,” said Shyam Prabhakaran (University of Chicago, Chicago, USA), senior author on the study, which has been published in the Journal of the American Medical Association (JAMA). “For every 15 minutes that pass without treatment, prior research shows there is a steady decrease in the chances of good outcomes for stroke patients. Getting to the right hospital quickly can be lifesaving.”

Our study indicates that low preoperative PNI is significantly associated with a higher incidence of ischemic stroke in patients undergoing non-cardiac surgery. Preoperative PNI, as a preoperative nutritional status evaluation index, is an independent risk factor useful to predict perioperative ischemic stroke risk, which could be used as an intervenable preoperative clinical biochemical index to reduce the incidence of perioperative ischemic stroke.

Brainomix’s e-Stroke imaging technology uses artificial intelligence (AI) to automatically process CT and MRI scans and alert doctors in real-time about those patients who would benefit most from mechanical thrombectomy (MT), a life-changing treatment which can reduce disability and prevent or limit long-term care needs in patients with the most severe strokes due to blockage of a large blood vessel supplying the brain. Prompt recognition of this type of stroke and transfer to specialist centres for treatment is vital to ensure best possible outcomes for patients.

On August 3, 2023, the APEX TRA SYSTEM™ radial artery access guidance system, independently developed by NeuroVita, a subsidiary of Sinomed, has been approved for marketing by the National Drug Administration of the People's Republic of China (NMPA). In October 2022, the Chinese Expert Consensus on Transradial Artery Access for Neurological Intervention was released, recommending that transradial access has higher safety, and patients do not need to be bedridden after the procedure, which can significantly improve patient comfort, reduce bedridden complications and nursing workload, shorten hospitalization time, and reduce hospitalization costs, so it is being more and more widely used in the field of coronary interventional diagnosis and treatment. However, the lack of specialized instruments is an important factor limiting the development of transradial artery approach neurointervention. At present, although the procedure can be completed by using conventional instruments to establish access via the radial artery, it is time-consuming to establish the access, the system support is insufficient, and the catheter of the access is easy to be broken during the procedure, etc. There is an urgent need to develop a specialized instrument for transradial nerve intervention. Therefore, this transradial coaxial system will further simplify the surgical operation, improve the safety and stability of transradial treatment, shorten the treatment time of transradial surgery, and reduce the incidence of complications.

Ischemic strokes are relatively frequent in Morocco. Their management generates an important cost, which is influenced by several factors such as severity of the disease and the duration of hospitalization. This cost can be decreased by rationalizing the expenses and acting on various risk factors of ischemic strokes.

The surface of this coated flow-diverter dense mesh stent independently developed and manufactured by NeuroVita adopts a unique ultra-thin anti-thrombotic coating design, which has a high bonding strength with the substrate and possesses the advantages of hydrophilicity and anti-thrombosis, and it is the first dense mesh stent with an anti-thrombotic coating design in China at present. In the present clinical trial, only a single antiplatelet drug was used in some of the cases after the operation, and we are full of anticipation for the results of the followup.

The Hmong people are an ethnic group of clans primarily from mountainous areas in Laos, China, Vietnam and Thailand. Many fought alongside the United States during the Vietnam War. Persecution from that alliance forced thousands of Hmong to flee Southeast Asia in the mid- to late 1970s, with many coming to the U.S. as refugees. The U.S. Hmong population now hovers at an estimated 368,609, according to 2021 census data.

Medical treatment is the mainstay of most cases of ICAD, whether asymptomatic or symptomatic. However, for patients who have failed medical treatment, endovascular revascularization can be performed with a low risk in most subgroups. Patients with hemodynamic compromise and large vessel emboli presentation appear to have the most potential benefit for intracranial artery stenting. The primary long-term risk for these patients post-stenting is in-stent re-stenosis. The use of DCB and DES has demonstrated lower re-stenosis rates, but shown mixed results regarding periprocedural complications. Currently, there are no FDA-approved DCB or DES devices for ICAD. The use of stenting as a rescue therapy in LVO thrombectomy has also shown promising early results. The investigation of this option in failed revascularization during thrombectomy may be beneficial to patients with AIS.

Elon Musk’s Neuralink isn’t the only company making progress on connecting people’s brains to computers. In April and May, surgeons at West Virginia University placed thin strips of a cellophane-like material on the brains of three patients. Made by New York-based startup Precision Neuroscience, the thumbnail-sized strips are designed to conform to the surface of the brain without damaging its delicate tissue.

The measured expenditures varied considerably among these studies with a global average cost from €610-€220,822.45. Given the great variability in the costs in different studies, we can conclude that we need to define a common system for assessing the costs of strokes. Possible limitations are related to clinical choices exposed to decision rules that trigger decisions alerts within stroke events in a clinical setting. This flowchart is based on the guidelines for acute ischemic stroke treatment but may not be applicable to all institutions.

Advances in stroke care policy during 2021 remained influenced by the COVID-19 pandemic, but several notable papers called attention to the costs of stroke care or the cost-effectiveness of options for care. Despite the variation in how costs were measured, estimated, and reported, policy recommendations for resourcing stroke services to support guideline-based care were found. The availability of cost data associated with all facets of stroke care and different perspectives (patients, hospitals, and payors) is increasing, but there are advances still to be made (Figure). This synthesis of a selection of papers published in 2021 on costs and stroke care effectiveness highlights the most recent efforts in examining the intersection of stroke care delivery and financial implications that could inform policy. The attention to appropriately financing stroke care delivery and its infrastructure for optimal quality and outcomes needs to remain at the forefront for decision makers who are responsible for allocating resources.
