Multiple Sclerosis (MS) is one of the leading causes of non-traumatic neurological disability among young adults. Due to its complex pathology and the lack of reliable disease models, there are no effective therapies for MS to prevent neurodegeneration or promote neuroprotection, and hence stop disease progression. The emergence of induced pluripotent stem cells (iPSC) has allowed the generation of patient-specific neural cell types for disease modelling, drug screening, and cell therapy.

For decades, multiple sclerosis (MS) has been depicted as a disease of the immune system running amok and attacking the whitish myelin sheath that encases the axons of the neurons in the central nervous system. The myelin is commonly analogized to insulation of electrical wiring; once compromised, the signals traveling through those axons get garbled and people with MS experience a range of neurological symptoms. In this telling, directionally MS is an “outside-in” condition of the immune system compromising the central nervous system.
But there is a camp of MS researchers who want to flip the narrative — or at least get their version considered — and have MS viewed as an “inside-out” phenomenon starting inside the central nervous system and resulting in secondary inflammation.

People taking a drug cocktail containing one of the body’s natural immune chemicals called interferon beta 1b plus a combination HIV drug and an antiviral drug took seven days to recover from COVID-19. In comparison, it took people on only the HIV combo drug 12 days to get better, researchers report May 8 in the Lancet. Recovery included reduced shedding of virus, improvements in symptoms and discharge from the hospital.

Because some oral treatments, such as cladribine, have long-lasting effects on the immune system, the cumulative effects of sequential monotherapies can resemble the effects of a concurrent combination therapy. This treatment scheme might lead to higher efficacy but also to new safety concerns. These sequential treatments were largely excluded in phase 2 and 3 trials; therefore, monitoring both short-term and long-term effects of sequential disease-modifying therapies in phase 4 studies, cohort studies, and registries will be necessary.
