Microbial Signature of Crohn's Disease Revealed

The identification of specific bacteria that are present—or not—in patients with the debilitating bowel disease could yield better diagnoses and therapies.

The largest study of its kind has revealed for the first time the specific gut bacteria involved in Crohn's disease, which afflicts an estimated 700,000 Americans with chronic, painful diarrhea and bleeding, among other unpleasant symptoms. With a better understanding of how the microbial ecosystem changes in children who suffer from the disease, researchers have gained clues that could someday lead to better diagnosis and treatment.

Microbiota-Dependent Crosstalk Between Macrophages and ILC3 Promotes Intestinal Homeostasis

Gilbert C FAURE's insight:

authors summary: The intestinal microbiota and tissue resident myeloid cells promote immune responses that maintain intestinal homeostasis in the host. However, the cellular cues that translate microbial signals into intestinal homeostasis remain unclear. Here, we show that deficient granulocyte macrophage colony-stimulating factor (GM-CSF) production altered mononuclear phagocyte (MNP) effector functions and led to reduced regulatory T cell (Tregs) numbers and impaired oral tolerance. We observed that RORγt+ innate lymphoid cells (ILC) are the primary source of GM-CSF in the gut and that ILC-driven GM-CSF production was dependent on the ability of macrophages to sense microbial signals and produce interleukin (IL)-1β. Our findings reveal that commensal microbes promote a crosstalk between innate myeloid and lymphoid cells that leads to immune homeostasis in the intestine.

PubMed comprises more than 23 million citations for biomedical literature from MEDLINE, life science journals, and online books. Citations may include links to full-text content from PubMed Central and publisher web sites.
Gilbert C FAURE's insight:

Stool samples were collected prospectively on transplant days 0, +5, +10 and +15 and analyzed by ELISA. Lower levels on day +5 (calprotectin and β-defensin-2) and day +10 (calprotectin, β-defensin-2 and sCD8) were associated with subsequent AGVHD. The most striking difference was with calprotectin on day +10. Patients with levels below 424 mg/kg had an incidence of 77.8 %, while those with levels above this threshold had a cumulative incidence of 0 % (p=0.002). Relative defects in gut mucosal immunity may be important in the pathogenesis of acute GVHD.
