For those interested in medical history, I really recommend, if you have not read it yet, that you read Everything Is Tuberculosis: The History and Persistence of Our Deadliest Infection by John Green. It's written like a story, it's gripping, and it will teach you a lot about tuberculosis.

Vaccination en France: quelques progrès ponctuels mais de nombreux angles morts.
Le rapport que vient de publier la Cour des Comptes sur la politique de vaccination depuis 2018, même s’il fait état de quelques progrès, nous montre que nous n’avons pas encore pris conscience qu’il s’agit d’une priorité de santé publique toujours négligée. Faut il rappeler que la vaccination a évité 154 millions de décès dans le monde?
1/ la France demeure un des pays les plus hésitants en matière de vaccination. Une publication récente place même notre pays dernier sur un échantillon de 67 pays! Entre 2000 et 2024 le pourcentage de la population favorable à la vaccination a reculé de 91,2% à 80,3%. Le constat n’est pas plus positif du coté des professionnels de santé; 1 médecin sur 5 estime que les enfants sont vaccinés contre trop de maladies, opinion rencontrée majoritairement chez les adeptes des thérapies alternatives! Les opinions erronées sur les adjuvants atteignent le même niveau chez les médecins que dans la population générale, seuls 43% se sentent à l’aise pour expliquer le rôle des adjuvants! Et pourtant malgré ce faible engagement des médecins le niveau d’exigence en matière de couverture vaccinale pour bénéficier de la rémunération forfaitaire liée aux objectifs de santé publique a été diminué. Comprenne qui pourra, le septicisme de certains médecins, même très minoritaires fort heureusement, étant un terreau particulièrement fertile pour entretenir les doutes de la population.
2/ l’exemple de la rougeole: encore considérée comme une maladie bénigne, l’OMS nous apprend que sur les 154 millions de décès évités, 93,7 millions l’ont été grâce à la vaccination contre la rougeole. Et le taux de vaccination en France, même s’il est en progrès demeure insuffisant pour éviter sa propagation. Il faut en effet une couverture vaccinale de 95% pour atteindre cet objectif, ce qui est loin d’être le cas dans de nombreux départements et notamment outre-mer.
3/ seulement 55% des jeunes de 15 ans sont vaccinés contre le papillomavirus, virus très répandu et à risque important de cancer, alors que de nombreux pays tels le Portugal, l’Espagne, la Suède arrivent à 80%.
4/ le taux de vaccination cotre la grippe, responsable de 9 à 10 000 morts par an, était de 54% pour la campagne 2024/2025, il est tombé à 50% un an plus tard alors que l’objectif affiché par l’OMS est de 75%
Les exemples de nos carences pourraient être multipliés notamment dans la population des adultes et adolescents. L’écho donné par la presse à ce rapport est inversement proportionnel à la place consacrée aux prétendus accidents vaccinaux.
Quand prendra-t-on conscience qu’il s’agit du levier le plus efficace pour améliorer notre santé pour un coût dérisoire?

New analysis out with the Edelman Trust Institute and my colleagues at the Yale School of Public Health, surveying nearly 13,000 people across 13 countries about how they decide what to believe about their health.

Trust in doctors hasn’t dropped. What has changed is the number of sources people draw from when making health decisions. AI, peers, online communities, and other sources are now competing for the same attention. Doctors haven’t necessarily lost their authority, but their voices are being diluted in an increasingly noisy information environment.
AI is already part of that shift: nearly half of people say someone with no medical training but good AI skills could match a doctor on at least one health task.

The people who hold the most divisive health beliefs are also the ones most densely connected to one another. And they are the group most likely to say it would matter if their healthcare provider had a presence in their community.

That matters because these aren’t necessarily people who have turned away from institutions. They are open to institutional voices that show up where they already are.
And what helps once you’re there isn’t complicated. Listen first. Welcome doubts without judgment. Take people’s concerns seriously. Our data suggest that having the right information isn't the only thing that matters. Showing up in communities and building relationships is valued most by the people who are hardest to reach.

The report closes with a case study of what this can look like in practice. After the East Palestine train derailment, a Yale researcher and a local activist went door to door together, asking residents about their experiences with water quality. That partnership became part of a $10 million NIH-funded research program.

Nobody’s beliefs had to be corrected along the way. The collaboration was organized around a shared goal, rather than agreement about what to think.

This is the first output of a larger collaboration between the Yale School of Public Health and the Edelman Trust Institute, and I’m glad to be part of it. Measles is contagious during its initial phase and for three to four days after the rash begins. However, most cases are benign and go unreported.

Before mass vaccination was introduced, nearly everyone contracted measles by age 15 and obtained lifetime immunity.
