โThe 15-Month Drop-Off: Addressing Health Literacy and Reminder Gaps in Bangladeshโs MR Vaccination Schedule
โโOn my busy night shift at the triage and emergency of the country's largest measles-dedicated facility, I have seen only 2 patients out of 53 who received their vaccines properly according to the EPI schedule. As a frontline clinician battling measles in pediatric patients, this hurts my heart as a mother. It shows how ignorance and a lack of care regarding vaccination have led our children to this immense suffering.
โWhen I think about the reason why we are here today, I feel the real problem is not only shortage of vaccines, but also the major drop-off in parents completing the EPI schedule,especially when it comes to the final two doses at 9 and 15 months, which are actually the MR vaccines for Measles and Rubella.
โWhy Caregivers are Missing the 15-Month Mark?
โWhen analyzing the patients in our measles ward, it is clearly visible that parents are simply not aware of the different diseases their children are being vaccinated against. They routinely take the first few vaccines up to 6 months of age, but they do not place the same emphasis on getting the last doses at 9 and 15 months. โBecause they have taken a few vaccines within 6 months, many parents think they have already given "enough" vaccines. They assume it is not that important to take all of them or finish the last ones. They are completely unaware of what vaccines they have taken so far and which ones are left.
โThe Strategy: Better Counseling and Automated Reminders
โGiven this scenario, I believe we can significantly raise the compliance rate by implementing two targeted strategies:
1.โComprehensive Counseling from Day One: If parents were properly counseled on the very first vaccination day about the full 15 months immunization journey, the compliance rate would rise. They need to be explicitly told the names of the diseases, the total number of vaccines required, and the severe complications that can occur if they do not vaccinate their children.
2.โDigitalized Vaccine Reminders: We must digitalize the vaccination program to arrange automatic reminders. By tracking the child's date of birth or logging data from the vaccination center where the last vaccine was given, the system could automatically remind the parents every time a vaccination date is near. This simple step would create a huge rise in adherence rates.

Abstract Since the inception of the World Health Organization (WHO) Extended Programme on Immunization in 1974, vaccines have saved the lives of approximately 150 million children, and they continue to save 2โ5 million lives each year, according to WHO estimates. The consensus around the benefit and funding of childhood vaccination in low- and middle-income countries has unraveled, and the hard-won gains of the past quarter century are at risk. At the same time, in some high-income countries, vaccine hesitancyโsometimes enabled by government policy pronouncementsโhas led to reductions in vaccination, resulting in outbreaks of measles in countries that had eliminated this disease. Despite these setbacks, advances in vaccine platform technologies, new immunogens, and new and expanding target populations have the potential to extend the direct and indirect effects of vaccination across the life course. In this Perspective, we discuss how each of these advances could shape the future vaccine landscape. We also highlight cross-cutting issues, including those related to equity, manufacturing, funding and vaccine hesitancy, which make successful implementation more complex.

How can public health compete with misinformation?
Thatโs the question Professor Matt Motta and I explored through our latest Boston University School of Public Health project: testing an influencer-based strategy to counter weight loss supplement misinformation on social media.
Thanks to the Harvard T.H. Chan School of Public Healthโs Center for Health Communication and its Health Content Creator Program, we recruited 7 diverse creators with a combined reach of nearly 2 million followers. We provided creators with an evidence-based digital toolkit and gave them full creative freedom to deliver science-informed content in their own voice. The result? More accurate, engaging health messages that reached audiences public health often misses.
Key takeaways: - Social mediaย canย be a credible public health tool when we workย withย creators, notย againstย platforms. - Messages shifted: <1% of content mentioned the lack of FDA approval for supplements before our intervention. Afterward? Nearly 90%. - Mentions of serious risks (liver, cardiovascular damage) went from zero to frequent. - The model is scalable and adaptable across health misinformation domains, whether for mental health, climate-related threats, or food production and policy.

Sen. Ron Johnson (R-Wis.) released the FDA analysis on pediatric deaths tied to COVID shots that was touted by former FDA leadership but never made public.
Last November, former Center for Biologics Evaluation and Research Director Vinay Prasad, MD, MPH, issued a memo to staff claiming that at least 10 children died due to a COVID shot.
But the number in the analysis is lower, and there's much less certainty about the links between the vaccines and the deaths.

I woke up this morning to news that I could not file under someone else's problem. The Zambian government withdrew support for and effectively canceled RightsCon days before it was due to open in Lusaka, with more than 5,000 participants already en route.
RightsCon is the world's largest gathering on human rights and technology, with a strong SRHR program this year covering censorship of reproductive health content, surveillance of people seeking abortion care, and disinformation targeting trans health.
The cancellation landed in a country where same-sex activity remains criminalized and SRHR information is already severely constrained. The reason given: "full alignment with Zambia's national values." The government did not need to say more.
My reaction was not to file this under digital rights. It made me think about the language circulating around health information in global health right now.
"Misinformation management." "Information integrity." "Quality health information." These terms are in our toolkits, trainings, and funding proposals. They all share the same assumption: institutions define what counts as accurate or trustworthy, and communities receive it. Institutions find that assumption very useful.

As the world saw with Covid-19, the only thing that spreads faster than a virus is misinformation, and hantavirus is no exception.
Despite only a few confirmed cases, conspiracy theories have swirled on social media, falsely claiming hantavirus is a planned pandemic, or a ploy to disrupt the US midterm elections.
Others allege it's a "bioweapon" created by big pharma to "poison" people or even that it's a side effect of the Pfizer Covid-19 vaccine.

LinkedIn has been hijacked by anti-science and anti-vaxxers. And such is unacceptable, and intolerable. The science here is sound, the publication of high scientific quality, and the data is clear. And the post is a direct quote from the paper itself. 359 individuals have seen this posts with only positive reactions from 13 individuals. The CIDRAP analysis is clear in its results: COVID has not killed dying elderly individuals, it ripped 28% of those of 5 years of life during the first 2.5 years of the COVID crisis. It is not because the CDC/FDA/ RFK Jr anti-COVID mindset has stopped the publication of a paper that has demonstrated the efficacy of the COVID vaccine, that we should deny this reality. And LinkedIn should not be polluted by those anti-science and anti-vaxxers that LinkedIn should removed posts about the efficacy of COVID vaccines. Doing so is unscientific, inaccurate, unacceptable, unethical, and criminally negligent. This post must be seen for what it is: the truth on COVID vaccines, whether anti-vaxxers like it or not. LinkedInโs policy is that posting against the efficacy of vaccines is not permitted. But posting the truth on such isโฆ. That LinkedIn tolerates the lies of McCullough Foundation, Children's Health Defense, Physicians for Informed Consent around vaccines is already going against their own policy. But to remove posts sound in science and with solid references is not what their policy is all about.

In this interview with Dr Peter McCullough, MD, MPH I reflected on the enormous risk that a doctor takes by challenging the prevailing narrative (link to full interview in comments).
The consequences extend far beyond losing a job, it can mean being unable to practice in the very profession where your expertise was built.
Thatโs why, in this context, the science must be perfect, if censorship occurs.
The personal, professional, and financial costs of being wrong are simply too great to ignore.
