Beginning with Edward Jenner’s discovery of the smallpox vaccine, the ever-expanding repertoire of vaccines against pathogens has saved many lives. During the COVID-19 pandemic, a revolutionary mRNA injectable vaccine emerged that effectively controlled the severity of disease caused by SARS-CoV-2. This vaccine induced potent antigen-specific neutralizing serum IgG antibodies, but was limited in its ability to prevent viral invasion at the respiratory surfaces. Nasal vaccines have attracted attention as a potential strategy to combat respiratory infections and prepare for future pandemics. Input from disciplines such as microbiology, biomaterials, bioengineering and chemistry have complemented the immunology to create innovative delivery systems. This approach to vaccine delivery has yielded nasal vaccines that induce secretory IgA as well as serum IgG antibodies, which are expected to prevent pathogen invasion, thereby diminishing transmission and disease severity. For a nasal vaccine to be successful, the complexity of the relevant anatomical, physiological and immunological properties, including the proximity of the central nervous system to the nasal cavity, must be considered. In this Review, we discuss past and current efforts as well as future directions for developing safe and effective nasal vaccines for the prevention of respiratory infections. This Review provides an overview of progress and future directions in the development of nasally administered vaccines for respiratory infections.

"Le cancer du col de l'utérus sera éradiqué au Danemark d'ici à 2040 grâce au vaccin contre le HPV, selon une étude" titre franceinfo de façon percutante en reprenant l'annonce des autorités sanitaires danoises qui peuvent légitimement se féliciter.
Mais ceci mérite qu'on y ajoute "les antivax sont des criminels", pour notre pays, ce qui aurait le mérite, peut-être, de faire réfléchir les parents encore hésitants ou les opposants les plus farouches à la vaccination préventive de leurs enfants.
Oui le Danemark peut se réjouir d'une prévalence des cancers du col de l'utérus soit descendue en-dessous du 10 cas pour 100 000 femmes. La combinaison d'un taux d'adhésion à la vaccination record de 89 % (le taux nécessaire à "l'immunité communautaire est de 90 %) pour les enfants, (garçons et filles) de 12 ans, avec un taux d'acceptation de 60 % du dépistage, la prévalence de ces cancers pourrait passer sous le seuil d'extinction de 4 pour 100 000 retenu par la World Health Organization à l'horizon 2040.
Les suédois pourraient bien atteindre ce but encore plus rapidement. Grâce au taux d'adhésion à la vaccination anti-HPV lancée en 2007, initialement destinée aux jeunes filles âgées d'au moins 13 ans puis étendue aux garçons avant d'être généralisée en 2020 , 82 à 83 % des femmes nées en 1999-2000 sont couvertes contre l'infection et les dommages causés par les HPV 16 et 18. Grâce à l'extension aux deux sexes, ce taux de couverture pour les jeunes nés après 2000 devrait être encore plus élevé. Mais grâce aux résultats des plus de 800 000 dépistages (pour un pays de 5,9 millions d'habitants !) effectués chez les femmes âgées de 23 à 64 ans entre 2014 et 2023, il est aujourd'hui possible d'établir que par rapport à une étude épidémiologique nationale réalisée en 1984, la prévalence des infections à HPV 16 et 18 a respectivement chuté de 96 et 99 % ! Au-delà de ce résultat déjà impressionnant, cette quasi-extinction des infections à HPV 16 et 18 dépasse le taux de couverture vaccinale permettant d'affirmer que la Suède a d'ores et déjà atteint le seuil d'immunité communautaire et devrait voir le cancer du col de l'utérus disparaître à quelques exceptions près.
Et la France qui se distingue comme championne d'Europe dans le classement des pays où les thèses antivax et complotistes sont les plus implantées, par rapport à la Suède et au Danemark, où en est-elle ? Eh ben... comme il fallait s'y attendre, c'est pas brillant malgré l'auto-satisfaction affichée dans les propos de nos amis du Ministère de la Santé ! Si le taux de vaccination progresse de 17 points pour atteindre 48 % entre 2022 et 2023 pour les collégiens de 12 ans, il faut surtout retenir que seuls 23 % des adolescents, filles et garçons, âgés jusqu'à 15 ans ont reçu au moins une dose de vaccin.
Et au regard de l'absence totale de visibilité de la campagne de vaccination 2024-2025, il est peu probable que ces chiffres progressent significativement, n'est-ce pas Santé publique France ? | 18 commentaires sur LinkedIn

To understand this, we need to define what a vaccine exemption is, and why they are needed.
An exemption generally means that someone can`t or won`t get a vaccine. Exemptions usually fall into three major categories: religious, philosophical, and medical.
Religious exemptions are based on the fact that a person`s faith and/or religious practice forbids vaccination."
Medical exemptions are reserved for those who have autoimmune disease, are on medication, have allergies to vaccine ingredients, or would otherwise be at risk from vaccination"
Some may look for a philosophical exemption when they feel that their belief system does not support vaccines
Medical exemptions are rare, because the criteria for being given a medical exemption are actually quite narrow Thus they are quite difficult to get

Now leaving the door opened to anti-vaxxers who will push lies of SIDS being linked to vaccines, despite it being a lie. Despite having been successful in reducing the number of babies that succumb to Sudden Infant Death Syndrome (SIDS), the National Institutes of Health has terminated its participation in the Safe to Sleep program, STAT News reported last week. The program encouraged parents to practice a suite of evidence-based safety practices when putting their infants to sleep, including placing them on their backs instead of their stomachs. It has been credited with a 50 percent reduction in SIDS deaths since its inception in 1994. Despite the progress, there is still more to do: In 2022, the last year for which there was data, about 3,700 babies died suddenly and unexpectedly, according to the CDC.

A patient of mine recently texted me asking if I had listened to Joe Rogan’s podcast episode where he interviewed Dr. Suzanne Humphries, a physician who wrote a book on vaccines and immunity. I had not listened to it, but I told him I would and that I’d circle back with him to have a conversation about it. And I did. I called him and we shared our viewpoints. Vaccines are a hot topic these days (and have been at multiple points in history, even in the 1800s when diseases were much more likely to kill you and your children). We had an honest conversation about what we know from the literature and what we don’t know. We talked about the process of decision-making and trade-offs when it comes to vaccinating against something. We talked about the loss of trust in our public health institutions during the pandemic. At least in my town, family doctors are accustomed to having patients who are either opposed to vaccines or prefer an alternative vaccine schedule. The pediatricians refuse to see them, so naturally they look for a medical home with family docs. I am not offended by differing viewpoints and don’t judge these conversations as a waste of my time. If my teenager came to me with an uncomfortable question about sex, I would not scoff and say they don’t need to be thinking about that stuff. If I don’t talk to them about it, they are going to find an answer of varying quality somewhere else. Similarly, if my patient asks me a question about a controversial topic or voices an unorthodox opinion about something, I am going to encourage an honest, unemotional conversation about it. It’s unemotional because my goal as their PCP is not to “win” them to my side. Rather, it’s to help them sift through the information so they can make the best possible choice for their health. In primary care, listening goes a long way. When my patients know that I am listening to them, that builds trust. Listening and trust are the foundation of a good doctor-patient relationship. When study after study shows the value of having a trusting relationship with your primary care doctor, well, conversations like this are how the sausage is made.
