Success with population health management requires significant attention to organizational processes and strategic goals, says KPMG's Wendy Vincent.
Some leading proponents of health care reform have argued that the goal of any health care system should be to deliver the most value to patients: the outcomes achieved for treating a medical condition relative to the costs incurred over a complete care cycle. But how do you calculate the value of care when, as is typically the case, a medical condition has many relevant outcomes, each measured in different ways? And how do you communicate that information in a way that is accessible and actionable?

The American Board of Internal Medicine Foundation intended for the Choosing Wisely campaign to support conversations between clinicians and their patients about what care is necessary.1 The campaign sought to do so by involving professional societies in creating lists of unnecessary tests and procedures and disseminating the lists to their membership. At the same time, the campaign partnered with Consumer Reports to produce consumer-friendly briefs of Choosing Wisely recommendations and to disseminate the briefs to consumers. It remains an open question whether clinicians or consumers at large are aware of specific Choosing Wisely recommendations or have changed their attitudes toward unnecessary procedures and tests.

En 2014, 559 300 personnes sont décédées en France, soit 1530 décès par jour en moyenne. Le nombre de décès est cette année inférieur à 11 000 de moins que les deux années précédentes. Le nombre de décès d’hommes est plus élevé que celui des femmes jusque vers 80 ans. Cette étude montre également qu’entre 1950 et 2014, le nombre de décès avant 60 ans est passé de 32 % à 12 %. Sur la même période, le nombre de décès d’enfants de moins d’un an a été divisé par 13.

Population health management requires a huge amount of effort and work, operating at scale, to drive results: calling patients to get them in for care, pre-registering and pre-certifying them before their appointments, ensuring optimal scheduling and referral patterns among physicians, and so on. Therefore, executives and administrators should seek partners that integrate services with technology—in short, partners able to deliver not just reports but measurable results. Working with these partners, health care organizations looking to succeed in the shifting reimbursement landscape should focus on four crucial tasks:
• Identify and engage patients in need of care
• Align physicians and care teams
• Create seamless transitions in care
• Optimize revenue and efficiency
What would make a smoker more likely to quit, a big reward for succeeding or a little penalty for failing? That is what researchers wanted to know when they assigned a large group of Consumer Value Stores (CVS) employees, their relatives and friends to different smoking cessation programs. The answer offered a surprising insight into human behavior. Many more people agreed to sign up for the reward program, but once they were in it, only a small share actually quit smoking. A far smaller number agreed to risk the penalty, but those who did were twice as likely to quit.
