Objectives Evidence-based decision-making is the sine qua non for safe and effective patient care and the long-term functioning of health systems. Since 2020 Digital Health Applications (DiHA) in Germany have been undergoing a systematic pathway to be reimbursed by statutory health insurance (SHI) which is attracting attention in other European countries. We therefore investigate coverage decisions on DiHA and the underlying evidence on health care effects, which legally include both medical outcomes and patient-centred structural and procedural outcomes. Methods Based on publicly available data of the Institute for Medicines and Medical Devices searched between 08/2021 and 02/2022, all DiHA listed in the corresponding registry and thus reimbursable by the SHI were systematically investigated and presented descriptively on the basis of predefined criteria, such as clinical condition, and costs. The clinical trials on DiHA permanently included in the registry were reviewed with regard to their study design, endpoints investigated, the survey instruments used, and whether an intention-to-treat analysis was performed. Risk of bias was assessed using the ROB II tool. Results By February 2022, 30 DiHA had been included in the DiHA registry, one third of them permanently and two thirds conditionally. Most DiHA were therapeutic applications for mental illness based on cognitive behavioural therapy. For all permanently included DiHA, randomised controlled trials were conducted to demonstrate the impact on health care effects. While medical outcomes were investigated for all of these DiHA, patient-centred structural and procedural outcomes were rarely investigated. The majority of clinical trials showed a high risk of bias, mainly due to insufficient reporting quality. Overall, the prices for DiHA covered by SHI are on average around € 150 per month (min. € 40; max. € 248). Conclusions Evidence-based decision-making on coverage of DiHA leaves room for improvements both in terms of reporting-quality and the use of patient-centred structural and procedural outcomes in addition to medical outcomes. With appropriate evidence, DiHA can offer an opportunity as an adjunct to existing therapy while currently the high risk of bias of the trials raises doubts about the justification of its high costs.

Physicians have been taking the Hippocratic Oath for centuries. The Oath contains a set of ethical rules designed to guide physicians through their profession; it articulates a set of true north principles that govern the practice of medicine. The Hippocratic Oath has undergone several revisions, most notably in 1948 by the World Medical Association. However, in an era of rapid change in medicine, we believe it is time to update the Oath with modest but meaningful additions so that it optimally reflects 21st century health care. The rise of digital health has dramatically changed the practice of medicine in a way that could not have been easily predicted at the time Hippocrates outlined his ethical principles of medicine. Digital health is a broad term that encompasses use of digital devices and platforms, including electronic health records, patient-provider portals, mobile health apps, wearable biosensors, artificial intelligence, social media platforms, and medical extended reality, to improve the process and outcomes of health care delivery. These technologies have driven a cultural transformation in the delivery of care. We offer modest suggestions to help prompt discussion and contemplation about the current Oath and its relevancy to our changing times. Our suggestions are not meant to be a definitive set of final recommendations. Rather, we propose new text that bodies such as the World Medical Association might consider integrating into an updated Oath, just as previous changes were adopted to ensure the Oath remains relevant and impactful for all physicians and their patients.

Health ministers and delegates from WHO/Europe’s 53 Member States have adopted the Region’s first-ever digital health action plan – an ambitious agenda that will leverage digital transformation in Europe and central Asia with the aim of improving people’s health and well-being. Meeting on 12 September 2022 at the 72nd session of the WHO Regional Committee for Europe, the ministers and delegates approved a resolution recognizing the critical role and potential of digital tools in the health sector, and building on the lessons learned during nearly 3 years of the COVID-19 pandemic. Digital health is among the 4 flagship areas of the WHO European Programme of Work 2020–2025 (EPW) – “United Action for Better Health in Europe”. The new action plan is a concrete step towards making the EPW a reality by leveraging digital tools to advance universal health coverage, protect people from health emergencies, and promote health and well-being in the Region. “Digital health should be seen as an enabler to achieve health goals, and not as the solution itself for health problems or needs,” explained Dr Hans Henri P. Kluge, WHO Regional Director for Europe. “To be meaningful and really promote better health, digital tools need good governance, proper legislation, and policies that promote the healthy use of these tools while providing the people who use them – health workers and patients – with the training and support they need to make the best of them.”Meeting health challenges with the help of digital tools The wider adoption of digital tools for health has real potential to help governments and people in the Region as they continue to work towards meeting existing health challenges, including those brought about by the pandemic. Digital health helps expand the concept of electronic health (eHealth) to cover areas such as:telemedicine – ensuring people can access health services regardless of where they live; health data and health information systems – ensuring authorities have the information they need to develop health policies; artificial intelligence and big data – supporting clinicians, providers and policy-makers in planning or implementing interventions; andcountering the online infodemic – helping people trust high-quality health information. The newly adopted action plan was developed following consultations with partners and the 53 countries of the Region. It takes into consideration countries’ priorities in these and other areas, as well as their needs and challenges, including issues with access to digital health services among vulnerable groups.Putting patients at the centre of digital solutions For digital solutions to truly succeed, the people using them need to be provided with the right training and knowledge.“WHO/Europe is here to support countries as they leverage the use of digital tools in an inclusive and transparent way, while also protecting people’s privacy and specific needs,” said Dr Natasha Azzopardi-Muscat, Director of the Division of Country Health Policies and Systems at WHO/Europe. Dr Azzopardi-Muscat continued, “The digital literacy of all users should be a key component of any successful digital health strategy. Together with governments in our Region, we will work on solutions that put the needs of patients and health workers at the centre.” The plan encourages countries to prioritize advancing digital health literacy to help achieve national health goals, improve health system performance, and guide future digital health investments and transformation. It proposes they do so by acknowledging the needs of citizens and health workers while promoting an integrated care approach that institutionalizes digital health in the Region.
