Digital Health Technology May Soon Solve These 5 Major Healthcare Challenges  #esante #hcsmeufr #digitalhealth #getready4it

Digital Health Technology May Soon Solve These 5 Major Healthcare Challenges  #esante #hcsmeufr #digitalhealth #getready4it | E-SANTÉ : COMPRENDRE. RELIER. TRANSFORMER. | Scoop.it

If you work in healthcare or medicine, then you know that the industry is facing some pretty significant challenges in need of solutions. The most significant of these challenges include:

  • Rising healthcare costs
  • Inefficient processes
  • Poor access to healthcare
  • Poor quality of care
  • And a lack of patient-specific treatment

These challenges are the culmination of decades of shifts within the healthcare industry and have the potential to dramatically reshape how the industry functions for both the patient and the practitioner. And while they are large challenges, thanks to the advent of new technologies, it appears that they could be resolved within the coming decade.

Below, we discuss these challenges, the technologies that may soon be used to address them, and the steps that you can take to begin to prepare to help tackle them.

Challenge #1: Reducing Healthcare Costs

People are collectively living longer than they have at any point in recorded history. While this is clearly a positive development brought about by tremendous medical advancements, it does also bring a challenge: The longer people live, the more medical resources must be devoted to them in order to support healthy and active lifestyles. This rising demand has increased the cost of healthcare for all individuals.

According to the U.S. Centers for Medicare and Medicaid Services (CMS), healthcare spending is expected to rise an annual average 5.5 percent from 2017 to 2026, at which point it will account for nearly 20 percent of the U.S. economy. In total, healthcare spending in the U.S. is expected to reach at least $5.7 trillion by 2026, driven largely by higher drug prices.

When it comes to solving the problem of ever-increasing healthcare costs, prevention is going to play a very important role. Preventing a chronic disease is, after all, much more cost effective in the long run compared to treating the disease. Though a prevention-first approach will take a multi-pronged plan of action in order to be effective, technology will likely play a large role.

Specifically, wearable technology—sensors allowing for the real-time monitoring of everything from activity level to blood pressure, heartbeat, and blood glucose levels—hold a lot of promise. By partnering the widespread adoption of these devices with data analytics, it may one day soon be possible to monitor patient health from afar and intervene directly to prevent these costly, chronic diseases from happening.

Challenge #2: Reducing Inefficiencies in Healthcare

American healthcare is a strange industry in the way that it utilizes both extremely high-tech solutions, like genome sequencing and augmented reality, and extremely low-tech processes, such as paper-based recordkeeping that must ultimately be digitized at a later date.

This isn’t just counterproductive; it’s wasteful. Administrative expenses play a large role in rising healthcare costs. In fact, it has been estimated that half of the money spent annually in the US on healthcare administration is ultimately wasted. Reducing these inefficiencies will be key to addressing many of the overarching concerns plaguing healthcare. One such process in need of modernization is patient record sharing.

When a patient moves from one hospital or physician to another, records must generally be physically copied and either delivered as hard-copy or faxed to the new provider, which can take time and often leads to incomplete records. This can be extremely hazardous to patients undergoing complex treatments, for whom complete records are essential.

There are a number of technologies being explored, however, which may one day be able to address these concerns. One potential solution may be using blockchain to store and share patient records. This could not only allow for a complete record to be maintained but also allow for secure transmission of patient records in a way that protects patient privacy and ensures the integrity of the data.

Challenge #3: Improving Healthcare Access

In order for a person to benefit from the major advancements in modern medicine, they first must have access to those advancements. Unfortunately, along both socioeconomic and geographic spectrums, healthcare access is not equitably distributed.

According to the Office of Disease Prevention and Health Promotion (ODPHP), access to healthcare can be broken into three separate issues:

  • Insurance coverage: Whether or not a patient is able to afford the costs to treat both acute and chronic illness
  • Geographic availability: Whether or not a patient is located somewhere where they have access to healthcare facilities and services
  • Timeliness of care: Whether or not a patient is able to address a medical concern in a timely fashion, whether it be a life-threatening injury or a slowly-developing chronic condition

Issues of geographic availability and timeliness of care are already being addressed through the use of technology today, through the use of smartphones, laptops, and tablets that can facilitate an appointment even when a patient cannot physically reach their doctor. The increased prevalence of telemedicine and e-visits allows patients to interact with caregivers virtually—a boon for those with limited mobility or those living in remote or hard-to-reach locations generally underserved by healthcare services. It’s likely that access to healthcare will continue to be addressed by advancements in and utilization of technology.

Challenge #4: Increasing Quality of Care

When it comes to healthcare, a lot of time is spent asking questions about increasing or improving “quality of care.” What each of these questions is really trying to answer is: How can we ensure that the patient gets the treatment and support that they need in order to have a positive outcome?

It’s an important question to ask, but one without an easy or clear-cut solution, largely because the solution often requires a multi-faceted approach. One thing that is clear, though, is that quality of care can be dramatically improved by healthcare providers always keeping their thinking patient-focused—an admitted challenge, especially for a healthcare system that is overly burdened and chronically understaffed. After all, how many patients can a single physician or team of nurses be expected to treat before the quality of care begins to suffer?

While addressing the shortage of nurses and doctors should be high on the priority list, until that is sorted out, it is likely that technology will play an increasingly important role as a safeguard against error that could have serious negative effects on patient outcomes.

The introduction of telehealth and virtual or e-visits, for example, may facilitate specialist consultations when a specialist is not available, as discussed above. Similarly, big data analytics, artificial intelligence, and machine learning may come to play important roles in identifying patients who are at higher risk of hospital-borne illness, post-operative infections, or other complications, enabling their providers to proactively preempt these complications from occurring.

Challenge #5: Making Medicine More Patient-specific

The advent of treatments that could be used to treat large segments of the population at once was a game-changer for the field of medicine. Broad-spectrum antibiotics and mass-produced pharmaceuticals ushered in the era of modern medicine in which we now live.

Unfortunately, these treatments also brought with them a number of serious consequences. Our overuse and over-dependence on broad-spectrum antibiotics, for example, has led to an increase in antibiotic-resistant disease, while our over-reliance on one-size-fits-all pharmacology has sometimes led to tragic and unforeseen consequences for some patients.

It’s for this reason that patient-specific treatment, drugs, and therapies are likely to be the future of medicine and pharmacology. For example, instead of using a broad-spectrum antibiotic to treat an infection, physicians are increasingly performing tests to identify the precise antibiotic that will be most effective in treating a disease. Instead of assuming a patient will react well to a certain drug, physicians are increasingly designing drugs to be patient-specific in order to ensure effectiveness—particularly important in the treatment of diseases like cancer.

This treatment is driven in large part by the emerging field of nanomedicine, which allows for patient-specific drugs and molecules to be created and deployed for more effective treatment. Also important are tools like wearable trackers and sensors, which allow healthcare providers to monitor the effects of their treatments, and the use of artificial intelligence, which can analyze massive amounts of data in order to determine which treatments will work best for a given disease or patient.

Preparing Yourself for the Healthcare of Tomorrow

Healthcare, like almost every industry, is rapidly changing thanks to the advent of new technologies. In order to stay competitive in the field and provide the best care for their patients, practitioners must do their part to stay aware of these burgeoning technologies and proactively seek the skills that will enable them to make use of the technologies as they enter the industry.

Who ‘owns’ the healthcare consumer of the future?  #hcsmeufr #esante

Who ‘owns’ the healthcare consumer of the future?  #hcsmeufr #esante | E-SANTÉ : COMPRENDRE. RELIER. TRANSFORMER. | Scoop.it

CVS and Aetna are merging. Amazon, JPMorgan Chase, and Berkshire Hathaway are forming a joint venture aimed at reducing health care costs and improving outcomes. Cigna is acquiring Express Scripts. The proposed mergers promise a revolution that could fundamentally alter the current healthcare landscape and the relationships between providers and patients. With these giant corporations betting big on healthcare, a logical question to ask is: What’s behind it all?

The answer is simple: ownership of the healthcare consumer experience, and by extension, the consumer.

Digitalization and healthcare consumerism

In the past few years, Amazon has reshaped the relationship between consumers and marketers. It has forged itself into being the preferred destination for consumers seeking convenience. Now, imagine Amazon applying this power to the healthcare sector. The company already offers a wide range of the over-the-counter drugs in their health and wellness section. Going from there to selling prescription drugs is not a big step. However, that step could become a big leap in terms of the shift in consumer loyalties if consumers are provided the option to order their 90-day medication refill and have it delivered to their doorstep (maybe even by a drone).

Recognizing the threat, many health systems are taking measures to digitalize their relationships with consumers by focusing on something they have long neglected: convenience. Virtual visits and e-visits are now becoming commonplace. A young mother of three no longer has to bundle her kids into the car and drive an hour each way to her hospital for a routine follow-up that takes all of fifteen minutes in the physician's office. A senior citizen in a wheelchair on multiple chronic-care medications no longer needs to "check in" by getting physically to a physician's office. They can both do their visits through secure messaging, or if required, through a virtual real-time consultation.

The above is just one example of how digitalization could reshape relationships between consumers and providers.

Data, analytics, digital

Here is another scenario that is already starting to play out. Consider a patient with high blood pressure. Technology is enabling patient-generated health data (PGHD) from wearables and sensors that include blood pressure, heart rate, glucose levels, and medication adherence to be transmitted seamlessly into the patient’s electronic health record (EHR). The combined data is being analyzed for trends and insights and made available to everyone involved in that patient’s care, enabling care teams to manage the patient more effectively. The patient can still control who can see the information by following an e-consent process through an app right on the mobile device. If a patient opts to participate in clinical studies, that person can be matched automatically with relevant opportunities. Both the individual patient and the population improve their health outcomes as a result.

Data-driven advancements are arriving in the form of both precision medicine and healthcare consumerism. Advancements in precision medicine are expected as the relationship between data from wearables, sensors, social determinants and other emerging sources is better interpreted through advanced artificial intelligence (AI), and yielding better outcomes. We are in the early stages of a new push toward patient-centered, consumer-directed care that is demonstrating strong growth potential.

What we are also starting to see is some redistribution of the in-person visits between traditional providers and emerging ones. As an example, CVS and Aetna are betting that consumers may prefer to visit one of their many walk-in clinics for minor conditions instead of waiting to schedule an appointment with their primary care physician in the hospital down the road. Urgent care is already shifting out of hospitals, and in many cases, going virtual altogether. The rise of companies such as Teladoc and Doctor-on-demand is clear evidence of this.

Bricks and mortar is not going away

None of this suggests that the traditional healthcare setting is fading into obscurity. Health systems, especially those with strong brands in their local and regional markets, have an unassailable lead today as trusted healthcare partners in their communities. Many of them are already making big investments in digitalization programs that will enable consumers to get the best of both worlds, namely a virtual experience for routine healthcare and urgent care needs, and an in-patient experience for acute care needs. For a high-quality patient journey, these two worlds must be tightly integrated. Only traditional hospitals can provide that truly integrated experience today.

The future of healthcare consumerism is not an either/or

If Big Data’s relationship to precision medicine has been on a more or less predictable trajectory, the explosive growth of healthcare consumerism has opened up options for healthcare consumers seeking convenience in addition to the quality of care. The healthcare leaders of tomorrow will ideally sit at the intersection of these two critical aspects of healthcare delivery. A reputation built on high-quality care alone will no longer be enough; neither will a reputation for slick user interfaces and transactional convenience.

A friend, who is also the CIO of a large health system, recently suffered a heart attack during a race. As he felt it coming on, he wisely checked himself into the medical tent from where he was rushed to a nearby hospital. A stent was put into his chest that saved his life. Imagine a scenario where with the help of real-time, advanced analytics and AI technologies, his imminent heart attack could have been predicted. Imagine, too, if he had not had access to one of the most experienced and qualified cardiologists in the country to attend to him during the emergency. Digitalization can ensure that the cardiologist's knowledge, wisdom, and experience are still accessible no matter the current physical location of the patient.

Ownership of the healthcare consumer experience requires a mixture of convenience and quality enabled by a robust data and analytics capability. No one sits precisely at that happy intersection today. However, the race is already underway to get there.

Global views on healthcare in 2018 #hcsmeufr #esante #digitalhealth | E-SANTÉ : COMPRENDRE. RELIER. TRANSFORMER. | Scoop.it

In the new Ipsos Global Advisor survey, Britons are most positive globally about their healthcare system and - with Spain - have the highest confidence that they get the best treatment.

Some of the remarkable findings:

  1. "Consumers’ assessment of the quality of their healthcare varies widely across the 28 countries covered in an Ipsos Global Advisor survey of more than 23,000 adults. Globally, 45% rate the quality of the healthcare they have access to in their country as good, 33% rate it as neither good nor poor and 23% as poor.
  2. Among problems facing healthcare systems, access to treatment/long waiting times is the one consumers around the world are most likely to view as a major concern (selected by 40%), most of all in Poland (70%), Serbia (68%), Hungary (65%) and Chile (64%). Insufficient staffing is the second most cited major problem globally (by 36%), most of all in Sweden (68%), France (67%), Hungary (63%) and Germany (61%). Cost of accessing treatment third most cited problem worldwide (by 32%) – but ranks at #1 in the U.S. (64%), Malaysia (49%), India (44%), Saudi Arabia (32%), and in a statistical tie with other issues in Australia (38%).
  3. Views about the future of healthcare differ widely across countries as optimism prevails in emerging countries (especially China, India, Saudi Arabia, Malaysia and throughout Latin America) while pessimism dominates in many developed countries (especially in Western Europe).
  4. Globally, less than half of all participants agree that the provider they last saw know them as a person (36%), or that they know that doctor very well (37%), or that the doctor showed them emotional support (40%).
  5. Globally, one half (49%) agree they get all the medical care they need while one quarter (24%) disagree and another quarter (27%) neither agree nor disagree.
  6. Globally, cancer ranks as the #1 public health concern with obesity a far second.

Thinking about Telemedicine

  • Globally, only 10% report having ever used telemedicine. Generally speaking, reported experience is far more common in emerging countries of Asia and the Middle East and in the United States than it is in Europe. Among the 10% who have used it, about two thirds say they would use it again and one third say they wouldn’t. Globally, 44% say they haven’t used it but would try it.
  • Globally, 12% say they currently use a connected health device or tool to manage their health, 15% say they have used one, but are not using any now, and 68% say they have never used one – the other 6% do not know. The five countries where usage is highest are the same as with telemedicine: China (28%), India (23%), Saudi Arabia (22%), Malaysia (18%) and the U.S. (15%).

In the perspective on patients/consumers there is still alot to do in getting healthcare right! A conclusion from the recent (2018) global survey of health by Ipsos.
"Views about the future of healthcare differ widely across countries as optimism prevails in emerging countries (especially China, India, Saudi Arabia, Malaysia and throughout Latin America) while pessimism dominates in many developed countries (especially in Western Europe)."

Interesting is to see how patients' responses do tell more about their beleifs, then about figures. Where is the global survey that really compare data like from GHO/WHO, EU, IFPMA, OECD, and Global Health 50/50?

More disturbing for scholars on healthcare is the disappointing percentage of people having experience with telemedicine!
