As the U.S. ages and more than 30 million new patients enter the health care system under the Affordable Care Act, experts predict that soon, there won’t be enough doctors for everyone who wants to see one—a shortage of 90,000 doctors by 2020, according to the Association of American Medical Colleges. To meet the demand, a surging class of almost-but-not-quite-doctors known as physician assistants, are stepping up to fill the M.D.’s shoes.

 

There are now more than 100,000 certified physician assistants in the U.S., up 34% since 2008, and while the employment prospects for most fields remain bleak, the Department of Labor forecasts the number of physician assistant jobs will grow another 30% by 2020. The number of nurse practitioners, which have similar training and duties, is also expected to rise.

 

Though physician assistants don’t go to medical school, experts say they can fill about 80% of a doctor’s role..

J.Kleinke pictures how health insurers company in the States respond to regulations with "horizontal consolidation", which will further reduce copmetition and the pressure to lower medical costs, and are also buying "vertically" pruchasing hopsitals and physicians pratices..with much happy talk about "care integration".

If health insurers wanted to protect their shareholders long term under the new Obamacare order, they would invest not in these desperate fantasies of market control, but in their own operations. “Managed care” may have come and gone but the myriad problems in the medical delivery system it tried to fix have not.

They would be better served minding their real business: managing the medical costs and health status of those they insure. There are no fancy tricks or world-beater press releases for this – just lots of hard work adjudicating claims, rooting out fraud, aligning payment with evidence, managing provider and patient adherence to the evidence, tracking outcomes, and steering people toward the better hospitals and doctors. These are the mostly untried but still true great ideas from the past two decades that everyone likes to talk about, but almost no one gets around to executing.

Entrepreneurs need to develop technology that would stop doctors from practicing like “voodoo doctors” and be more like scientists.

Health care must be more data driven and about wellness, not sick care.

Eighty percent of doctors could be replaced by machines.

Khosla assured the audience that being part of the health care system was a burden and disadvantage.

I recently viewed health care through the lenses of a technology entrepreneur by attending the Health Innovation Summit hosted by Rock Health in San Francisco. As a practicing primary care doctor, I was inspired to hear from Andy Grove, former CEO of Intel, listen to Thomas Goetz, executive editor of Wired magazine, and Dr. Tom Lee, founder of One Medical Group as well as ePocrates.

Not surprising, the most fascinating person, was the keynote speaker, Vinod Khosla, co-founder of Sun Microsystems as well as a partner in a couple venture capital firms.

Entrepreneurs need to develop technology that would stop doctors from practicing like “voodoo doctors” and be more like scientists.

Health care must be more data driven and about wellness, not sick care.

Khosla assured the audience that being part of the health care system was a burden and disadvantage. To disrupt health care, entrepreneurs do not need to be part of the system or status quo.

Khosla believed that patients would be better off getting diagnosed by a machine than by doctors.

Is it possible technology entrepreneurs can disrupt health care? He challenged any doctor in the room to counter his points.

Silence.

Was it because everyone agreed? Were the doctors in the room simply stunned? Was there a doctor in the house? And where did he get that 80 percent statistic?...

The success of devices like the iPhone and iPad in healthcare has become so pronounced that the Department of Health And Human Services has begun to single-out the use mobile devices as part of the meaningful use requirements for electronic health records (EHR) systems. In addition to identifying mobile device use, the agency has also taken steps towards explicitly regulating mobile device security needs in the healthcare industry.


This week, the federal agency released the official set of stage two meaningful use requirements, which will go into effect in 2014. A large portion of the stage two requirements focus on mandating more frequent use of EHRs and electronic health information exchange systems. For example, Doctors will be expected use digital systems to order lab work and diagnostic imaging for at least 30% of their patients.


Meaningful use rules for stage two come in two different documents, the CMS and ONC rules. Both documents include frequent

references to smartphones, tablets, and mobile apps (the language is by and large generic and doesn’t specify mobile platforms like iOS and Android by name).


Some of the rules impact data security when stored on or accessed from a mobile device:

 

'Requiring health-care providers to assess whether they need to encrypt protected health information data while at rest, in reaction to multiple breaches of PHI from mobile devices.

 

The general policy we express in this certification criterion requires EHR technology designed to locally store electronic health information on end-user devices to encrypt such information after use of EHR technology on those devices stops

 

Encryption is required for mobile devices, and EHRs must be able to create an audit log of such encryptions if the system allows local storage of data on the devices.'

 

Other rules stipulate that some of the required use of digital systems can be met using mobile devices:


'Requiring providers to use computerized physician order entry – which specifically can include a mobile device — for 60 percent of patients and 30 percent of laboratory and radiology orders.

 

The provider’s use of computer assistance to directly enter medical orders… from a computer or mobile device.'


Still others describe how mobile technologies can be used for engaging patients and providing access to their records (stage two requires hospitals and practices to offer 5% of patients online access to their health data):

 

'[S]ecure email, a secure portal, even some type of mobile application could all be examples for secure messaging methods that could potentially meet this certification criterion.

 

Vendors may develop mobile patient-engagement apps using technology that meets basic requirements for EHR certification, including secure email, portals and mobile applications using a variety of transport standards.'

 

The frequency of the references and their specificity clearly highlights the increasing role that the iPhone, iPad, and other mobile technologies have in healthcare - on the part of doctors, nurses, and hospitals as well as on the part of consumers. A recent study notes that 40% of mobile apps being developed today are related to healthcare or general health and wellness topics.
