Access to healthcare is a requirement for human well-being that is constrained, in part, by the allocation of healthcare resources relative to the geographically dispersed human population1–3. Quantifying access to care globally is challenging due to the absence of a comprehensive database of healthcare facilities. We harness major data collection efforts underway by OpenStreetMap, Google Maps and academic researchers to compile the most complete collection of facility locations to date. Leveraging the geographically variable strengths of our facility datasets, we use an established methodology4 to characterize travel time to healthcare facilities in unprecedented detail. We produce maps of travel time with and without access to motorized transport, thus characterizing travel time to healthcare for populations distributed across the wealth spectrum. We find that just 8.9% of the global population (646 million people) cannot reach healthcare within one hour if they have access to motorized transport, and that 43.3% (3.16 billion people) cannot reach a healthcare facility by foot within one hour. Our maps highlight an additional vulnerability faced by poorer individuals in remote areas and can help to estimate whether individuals will seek healthcare when it is needed, as well as providing an evidence base for efficiently distributing limited healthcare and transportation resources to underserved populations both now and in the future. A global analysis generating high-resolution maps of travel time shows that 91.1% of the world’s population can reach a hospital or clinic within an hour if they have access to motorized transportation, but only 56.7% can do so by walking, highlighting additional inequities for underserved populations accessing healthcare.

COVID-19 has resulted in an increased demand for eHealth services globally. There is emerging evidence for the efficacy for group eHealth interventions that support population-based mental health and wellbeing, but a systematic review is lacking. The primary objective of this systematic review is to summarize the evidence for eHealth group counseling and coaching programs for adults. A second objective is to assess, within studies selected for our primary objective, the impact of programs that encourage PA on outcomes compared to those that do not. Randomized controlled trials that assess the impact of eHealth group counseling or coaching programs on mental health, health behavior, or physical health activity among community-dwelling adults will be included. We will search the following electronic databases (from January 2005 onwards): MEDLINE, PsycINFO, CINHAL, and the Central Register of Controlled Trials. The primary outcomes will be changes in mental health conditions (e.g., depression, anxiety, stress, quality of life), behavioral health conditions (e.g., substance use, smoking, sexual behavior, eating behavior, medication adherence), and physical health conditions (e.g., coping with cancer, menopausal symptoms, arthritis pain). Secondary outcomes will be changes in physical activity. Two reviewers will independently screen all citations, full-text articles, and abstract data. Potential conflicts will be resolved through discussion with a third reviewer. A narrative synthesis without meta-analysis will be conducted. The strength of the body of evidence will be assessed using GRADE. The risk of bias in individual studies will be appraised using the Cochrane Risk of Bias 2.0 tool. Potential sources of gender bias in included studies will be considered at all stages of the planned review. This review will contribute to the literature by providing evidence on the effectiveness of eHealth counseling and coaching programs delivered to adults in a group format. The protocol has been registered at the International Prospective Register of Systematic Reviews (PROSPERO: CRD42020187551

Training in digital knowledge and skills should become a new core component of training for future and current doctors in all Member States: this was agreed by representatives of students/junior doctors and deans, gathered at the initiative of the European Commission and Erasmus University Medical Center on 12 April in Rotterdam. A set of recommendations for action were developed at this meeting, and participants committed themselves to taking them forward in their own national context to raise policy makers’ awareness.

Artificial intelligence (A.I.) is expected to significantly influence the practice of medicine and the delivery of healthcare in the near future. While there are only a handful of practical examples for its medical use with enough evidence, hype and attention around the topic are significant. There are so many papers, conference talks, misleading news headlines and study interpretations that a short and visual guide any medical professional can refer back to in their professional life might be useful. For this, it is critical that physicians understand the basics of the technology so they can see beyond the hype, evaluate A.I.-based studies and clinical validation; as well as acknowledge the limitations and opportunities of A.I. This paper aims to serve as a short, visual and digestible repository of information and details every physician might need to know in the age of A.I. We describe the simple definition of A.I., its levels, its methods, the differences between the methods with medical examples, the potential benefits, dangers, challenges of A.I., as well as attempt to provide a futuristic vision about using it in an everyday medical practice.
