National Expenditure For False-Positive Mammograms And Breast Cancer Overdiagnoses Estimated At $4 Billion A Year

Populationwide mammography screening has been associated with a substantial rise in false-positive mammography findings and breast cancer overdiagnosis. However, there is a lack of current data on the associated costs in the United States. We present costs due to false-positive mammograms and breast cancer overdiagnoses among women ages 40–59, based on expenditure data from a major US health care insurance plan for 702,154 women in the years 2011–13. The average expenditures for each false-positive mammogram, invasive breast cancer, and ductal carcinoma in situ in the twelve months following diagnosis were $852, $51,837 and $12,369, respectively. This translates to a national cost of $4 billion each year. The costs associated with false-positive mammograms and breast cancer overdiagnoses appear to be much higher than previously documented. Screening has the potential to save lives. However, the economic impact of false-positive mammography results and breast cancer overdiagnoses must be considered in the debate about the appropriate populations for screening.

ABSTRACT

Overdiagnosis occurs when a true abnormality is discovered, but detection of that abnormality does not benefit the patient. It should be distinguished from misdiagnosis, in which the diagnosis is inaccurate, and it is not synonymous with overtreatment or overuse, in which excess medication or procedures are provided to patients for both correct and incorrect diagnoses. Overdiagnosis for adult conditions has gained a great deal of recognition over the last few years, led by realizations that certain screening initiatives, such as those for breast and prostate cancer, may be harming the very people they were designed to protect. In the fall of 2014, the second international Preventing Overdiagnosis Conference will be held, and the British Medical Journal will produce an overdiagnosis-themed journal issue. However, overdiagnosis in children has been less well described. This special article seeks to raise awareness of the possibility of overdiagnosis in pediatrics, suggesting that overdiagnosis may affect commonly diagnosed conditions such as attention-deficit/hyperactivity disorder, bacteremia, food allergy, hyperbilirubinemia, obstructive sleep apnea, and urinary tract infection. Through these and other examples, we discuss why overdiagnosis occurs and how it may be harming children. Additionally, we consider research and education strategies, with the goal to better elucidate pediatric overdiagnosis and mitigate its influence.

On 11 March, the UK’s House of Commons took a bold and exemplary step, voting into law legislation that will introduce plain packaging for cigarettes in England by May 2016. Following Australia and, by just a few weeks, Ireland, England becomes only the third country to open up this latest front in the battle against tobacco harm.

The decision was exemplary, says the global tobacco control campaigner Judith Mackay, because it will encourage other countries with fewer resources to follow suit. But it was bold because inevitably the UK will now be engulfed in a storm of legal challenges from an industry determined to fight for every inch of ground, even as its products continue to kill millions around the world (fig 1).

Doctors think lots of patients ask for medicine they don’t need. This study says that’s not true.

We know that wasteful spending is a huge problem in health care. Every year, the United States spends $765 billion annually (about one third of our overall health care dollars) on things that do not make Americans any healthier.

This is both a huge problem, and opportunity. If we could make a dent in unnecessary wasteful spending, cut it in half or even a quarter, it would be a huge reduction in health care costs. And that could happen without harming Americans: we'd be eliminating the care that, by definition, isn't needed.

But if we're ever going to make a dent in unnecessary spending, such on unneeded MRIs or too much medication we need to understand why there's all this wasteful spending happening in the first place.

A new paper in JAMA Oncology tries to clear that up: The study, which examined 5,050 patient-physician encounters, found that -- despite doctors' perceptions -- patients aren't demanding medicine they don't need.

Cow’s Milk Contamination of Huma Milk Purchased via the Internet

BACKGROUND: The US Food and Drug Administration recommends against feeding infants human abstract milk from unscreened donors, but sharing milk via the Internet is growing in popularity. Recipient infants risk the possibility of consuming contaminated or adulterated milk. Our objective was to test milk advertised for sale online as human milk to verify its human origin and to rule out contamination with cow’s milk. METHODS: We anonymously purchased 102 samples advertised as human milk online. DNA was extracted from 200 µL of each sample. The presence of human or bovine mitochondrial DNA was assessed with a species-specific real-time polymerase chain reaction assay targeting the nicotinamide adenine dinucleotide (NADH) dehydrogenase subunit 5 gene. Four laboratorycreated mixtures representing various dilutions of human milk with fluid cow’s milk or reconstituted infant formula were compared with the Internet samples to semiquantitate the extent of contamination with cow’s milk. RESULTS: All Internet samples amplified human DNA. After 2 rounds of testing, 11 samples also contained bovine DNA. Ten of these samples had a level of bovine DNA consistent with human milk mixed with at least 10% fluid cow’s milk. CONCLUSIONS: Ten Internet samples had bovine DNA concentrations high enough to rule out minor contamination, suggesting a cow’s milk product was added. Cow’s milk can be problematic for infants with allergy or intolerance. Because buyers cannot verify the composition of milk they purchase, all should be aware that it might be adulterated with cow’s milk. Pediatricians should be aware of the online market for human milk and the potential risks.

Quindicimila richieste d?aiuto in più al Pronto soccorso solo a gennaio, quando sono stati visitati 341 mila malati. L?influenza presenta il conto. Salato. In Lombardia non sono mai stati usati così tanti farmaci specifici come nei mesi di gennaio e febbraio di quest?anno. È un numero da primato: complessivamente sono state consumate dieci milioni di dosi in più rispetto al 2010 e quasi 4 milioni in più rispetto al 2011, l?anno dell?allarme per l?arrivo dell?influenza suina, l?H1N1.Negli ultimi giorni all?assessorato della Sanità sono stati fatti i calcoli dell?impatto economico dell?epidemia. Solo per il consumo di antibiotici e di farmaci per il sistema respiratorio sono stati spesi 5,6 milioni di euro in più nel giro di due mesi, con un aumento della spesa farmaceutica territoriale superiore al 10% rispetto allo stesso periodo dell?anno precedente. Il mese più terribile, gennaio: in soli 30 giorni c?è stata una crescita del 60% di antibiotici rispetto al gennaio 2010, l?anno preso come paragone dall?assessorato alla Sanità (15 milioni e 798 mila dosi contro 10 milioni e 872 mila).Tutta colpa di un virus più aggressivo che mai. Questo è l?anno del ritorno della suina. In una forma particolarmente cattiva. Nel momento clou si trovano a letto fino a 9 lombardi su mille. È il doppio rispetto all?apice della scorsa stagione influenzale.Alla virulenza dell?influenza si aggiunge lo scandalo del vaccino Fluad, che comporta una battuta d?arresto delle vaccinazioni.
