An updated economic evaluation was conducted to compare the cost-effectiveness of the four tumour necrosis factor (TNF)-α inhibitors adalimumab, etanercept, golimumab and infliximab in active, progressive psoriatic arthritis (PsA) where response to standard treatment has been inadequate.

To develop personalized medicine strategies for improvement of patient management in rheumatoid arthritis, the clinical and molecular properties of the individual patients need to be well characterized. A crucial step in this approach is to discover subgroups of patients that are characterized by a good or poor treatment outcome. Dennis and colleagues have identified distinct pretreatment gene expression profiles in affected synovial tissue specimens and a tissue type-related systemic protein pattern which are associated with a positive or negative clinical outcome to monotherapy with adalumimab (anti-TNFα) and tocilizumab (anti-IL-6 receptor). These observations assign biological pathways associated with response outcome and provide evidence for the existence of systemic, easy-to-measure predictive biomarkers for clinical benefit of these biologics.

Popliteal cysts and subgastrocnemius bursitis are associated with increased symptoms as well as radiographic and MRI-detected joint structural abnormalities. Longitudinal data will help resolve if they are a consequence or a cause of knee joint abnormalities.

In the preclinical phase of RA, IgG ACPA comes first, followed by IgA, and then by IgM. For RF, the IgG subtype seems to appear after IgA and IgM. As postulated previously [3], the IgG-dominated anti-CCP pattern might reflect a mature T cell-dependent immune reaction, whereas the IgM-dominated RF response might represent a non-isotype switched T cell-independent immune response. All investigated autoantibodies rise over time before RA diagnosis. Finally, smoking seems to influence both IgA APCA and IgA RF levels and this is in line with the hypothesis that smoking induces or augments a mucosa-related immune response in lungs.
