My take on QR codes are:
1 - Source: where did it come from? If you find one in a public place, be very careful. As opposed to a closed workshop in your place of work.
2 - Where does it go to? Check the URL it goes to. If it is meant to be a survey on your LMS, then it should start with that URL. If it does not, I would be careful about entering data.
