We present the use of finite element analysis and computer simulation based on cardiac CT in three patients with severe MAC in whom transcatheter devices were considered. In the first two cases, the computer simulations were performed after the clinical procedure and were concordant with the clinical outcome. For the third case, computer simulation was performed prior to the clinical procedure. This indicated unsuitability for transcatheter device deployment and a subsequent medical management was adopted.

The PACO trial is a randomized clinical trial conducted in Heart Centers of Kuopio and Turku university hospitals. The coronary artery disease, aortic valve stenosis and mitral valve insufficiency patients preparing for elective coronary artery bypass grafting (CABG), aortic valve replacement (AVR) or mitral valve repair (MVR) will be randomized to either intervention or control group. The specific operation groups (CABG, AVR and MVR) will be analyzed separately. The 7-day baseline measurements of sedentary behaviour (SB), physical activity (PA) and sleep will be conducted to the patients by using the RM42 accelerometer during the pre-operative month.

Transcatheter heart valve (THV) replacement is growing rapidly worldwide. Transcatheter aortic valve replacement (TAVR) is the treatment of choice in severe symptomatic aortic stenosis in patients considered to be at high and intermediate surgical risk. Building on the continued success of TAVR, transcatheter mitral valve replacement (TMVR) is rapidly evolving and several devices are under investigation. Multidetector computed tomography (MDCT), with its high spatial resolution and ability for multiplanar reformatting, has become the standard imaging modality in pre-procedural assessment.

Professor Jean-Francois Obadia, principal investigator, Civil Hospices of Lyon, France, said: "We show for the first time that despite reducing secondary mitral regurgitation, percutaneous repair of the mitral valve does not improve survival or symptoms, or reduce heart failure hospitalisations compared to standard medical treatment alone. This strongly suggests that this procedure is futile in patients with heart failure and secondary mitral regurgitation."

Studies show that in high-risk patients with symptomatic aortic stenosis, TAVI may be cost-effective compared with medical management (MM) across many health care settings. In contrast, studies of TAVI compared with surgical aortic valve replacement (SAVR) yield conflicting and inconclusive results. The limited data available show that TMVR may also be cost-effective relative to MM in mitral valve disease. Existing evidence indicates that transcatheter techniques may be cost-effective options, relative to MM, in high-risk patients with valvular disease.
