In case you hadn’t noticed, there’s a growing clamor for a global commitment to universal health coverage (UHC). You might have seen the recent special issue of the Lancet on “the struggle for UHC”.Inevitably, accompanying this clamor, there’s been a lot of wracking of brains on how to measure progress toward UHC. With the excitement of a new political agenda, there’s understandably a desire to carve out a new measurement agenda too.

Universal health coverage: Old wine in a new bottle? If so, is that so bad?

It's easy to see how the concept of universal health coverage (UHC) became so elusive.

At the start, the idea must have seemed straightforward enough. Lots of countries "covered" only part of their population, and several were making efforts to expand coverage to "uncovered" populations. China, for example, started out on this process in 2003, trying to expand coverage to the rural population that lost coverage when the old rural cooperative medical scheme collapsed following the de-collectivization of agriculture in 1978.

This week the World Health Organization held a major international meeting on universal health coverage (UHC), with Director General Margaret Chan reaffirming her regard for universal coverage “as the single most powerful concept that public health has to offer.” While the term “universal” signals that the entire population will be “covered,” an unanswered question is: covered with what? Another way to put the question: What health benefits or interventions would represent coverage, taking into account UHC’s implicit goals of improved health, equity and financial protection?

Health insurance is not a right, it's a personal responsibility and choice. If you need it you buy it and if you want it you choose to get it. If you can't afford it you are already covered under Medicaid or Medicare or through emergency medical treatment in our ERs. Even illegals can get free medical care in our ERs. While this is not ideal it is already offered to the poor and illegals. 

 

What we can do is plan for the future by taking care of ourselves and saving up for a rainy day. Health insurance is not the govts job to provide and it's not a 'right'. We have a personal responsibility to take care of ourselves, provide for our families and seek charity from private organizations if needed. It's not the job of the govt/other taxpayers to provide.
