This tragic incident occurred because a D shackle failed. I find it hard to imagine the terror of the men as the tower collapsed. Tragic safety incidents really drive home the need for safety fundamentals. A low grade D shackle, valued at  a few dollars, cost 7 workers their lives and the  total destruction of the project.

 

The OHS Discipline is perceived by many as bureaucratic ,a hindrance to efficiency and economically unsustainable.

The great challenge for the OHS Discipline is that good safety can only be measured by no incidents.

The cost of poor safety is only revealed when safety fails, it is only then  that the human costs really stack up.

 

When everyone goes home after work, safety has had a quietly successful day, seldom seen and an often unrewarded.

 

I love it.

I was looking for some good quad bike roll over clips, and found this one, the began thinking it relates to another safety issue which is also one of my favourites, the near miss.  Yes, the first thing we will notice in this clip is, that's right the gender, the less safety conscious of the species. You will also notice a lack of PPE.

  As I was watching this clip and saw time and time again near misses. Each time our friend has a near miss, which of course is ignored, an inevitable incident looms closer, until finally the near miss is a hit. 

 

The lesson for safety is that if ignored or unreported the near miss will become a more serious incident.

There is a light hearted clip that illustrates the dangers of angle grinders, it was great to see that the operator was wearing the correct PPE, which included a face shield, those of you with strong stomachs may wish to see what angles grinder incidents can do to humans.Do a Google images search for angle grinder injuries- be warned it is not for the faint hearted there are some pretty graphic images.

 

I have always toyed with a safety road show called the Safety Rocky Horror show. I would like to be involved in a future career that uses different mediums to promote safety awareness in the workplace.

Triggering E36 Rollover Protection System [ROPS]

This is safety innovation at its best , using existing air bag technologies in a new application. This application of technology, if applied to quad bikes could save up to 20 lives a year in Australia alone. The real question we should be asking is why does it take so long for the manufactures, legislators and us the consumers to do anything about it. There have been 4 fatalities so far  this year.  ROPS or, Roll Over Protection Systems,  if fitted to all new quad bikes would over over a period of time  dramatically reduce this figure.

 

Safety should be innovative and exciting, that is what attracted me to the career.

This takes safety to new heights. What kind of emergency rescue plan would be need to in place , and how would the emergency rescue plan be practiced. Just viewing the footage gives me the creeps. How fast can workers descend from the top in poor weather? How will a disabled worker caught on the tower be rescued?

 

I will not be pursuing a career in this high risk business.

This article inspired me because most of my working life has been in the third world and I believe the best way to advance woman's rights and liberties is for them to work, that is why I have selected this article.

  Safety triumphs in a collaborative consultative environment.  Good on Tata, Dupont and the woman involved. It is inspiring to work in the safety discipline when one  sees how the application of the fundamental principals of worker consultation and involvement leads to worker cooperation and worker ownership of their own safety, which in turn drives effective positive change for all the female workers involved.

I worked in agriculture for 20 years and agricultural safety matters still get my attention. Agro chemicals are a large part of modern horticulture, whilst awareness of the active ingredient has grown this article is interesting in that it raises the lid on the often overlooked ingredients in many product formulations. As an OHS professional we need to consider what solvents  and other chemicals are used in the product formulation. Very often it is not displayed on the products SDS. For example a certain product may have a 35% active ingredient formulation,  what is the make up of the  other 65%. Modern chemicals are part of our daily lives, and our society has partly become possible due to the use of agricultural and other chemicals in our workplaces.

The point  I am making is that OHS Professionals may at times need to look beyond the data  presented to the less obvious.
