Chronic Care: A Systematic Review of the Literature on Health Behaviour Change for Chronic Care

A systematic review of health behaviour change in chronic care. 

 

This review identifies, appraises and summarises the most effective evidence-based theories and programmes of health behaviour change for individuals with diabetes, chronic obstructive airways disease, asthma, stroke and hypertension. Social Learning Theory was the most widely-used, effective, health behaviour change theory. Some target behaviours were improved in four of the target chronic conditions (diabetes, asthma, hypertension and non–disease specific). Cognitive Behavioural Theory/Therapy (as reported in COPD and hypertension), Transtheoretical Model (as reported in hypertension and non–disease specific) and Self-regulation (as reported in diabetes and asthma) were also effective health behaviour change theories.

Have you ever tried to make and sustain a lifestyle or behaviour change? How easy was it? Someone once explained to me that some things in life are simple, but that doesn’t equate to them being easy. For example, if you want to lose weight, it’s simple – exercise more and eat less, but if you’ve tried to do this, it ain’t easy! So how can healthcare professionals assist those with long-term conditions like osteoarthritis (OA) and rheumatoid arthritis (RA) to make health-related behavioural changes, when required? A review article by Knittle et al (2012) tries to address this with a useful overview of current theories of behaviour-change with evidence based suggestions for clinical practice.
