Limiting use of home care devices depends of cost and unfunctional design. They are either too complicated to use or bring bulky devices at home.

ZHEN MINIMALISM and mobile easy-to-use devices are of paramount importance in the Health 4.0 environment of anytime-anywhere-anything-anyhow health care and social care follow up. Integrated care transversal units and PERSONAL ROBOTS will help us at any time in the near future.

We have just to design and choose adequate devices.

At the beginning of 2017 appeared the publication tracking “Physiomes” , thank to wearing biosensors and reveal unexplore useful health related information. It determines body adaptation to circadian changes, inflamation, or other external influences such as barometric pressure etc… They are capable to measure the “Change of Heart-COH” and understand body changes that brings it particularly adapted to frail elderly people. The main concern on Silver Age and Chronic conditions is to achieve HRQoL (health related quality of life). The Commission started an open tool MAFEIP (http://is.jrc.ec.europa.eu/pages/TFS/MAFEIP.html ) to evaluate AHA (Active Healthy Aging). Those trials require an holistic vision for evaluation that incorporate all type of integrated surrounding data (weather, food, cooking, water, social...) mixed with small personal data obtain with forgettable or forgetting wearables non-intrusive for users particularly those in the silver ages. And require the human touch of Guardian health angels available for support and intervention in charge of controlling Small Data.

But still is not solve where the data goes and who is the data owner. This is HSD (Health Small Data) and should belong to the patient or the user in general, should not be used without owner permission and in any case for the owner's benefit. Any other conditions require a serious contract for SLA (Service Level Agreement ) on which data use, storage and manipulations should clearly stated.

As today published in Nature there will be a sensor array for multiplexed in situ perspiration analysis, which simultaneously and selectively measures sweat metabolites (such as glucose and lactate) and electrolytes (such as sodium and potassium ions), as well as the skin temperature (to calibrate the response of the sensors).

The accurate measurements of the device assure a new development of metabolites detection in sweat, that is an ultrafiltrate of the blood and therefore could be used for permanent measurement in real time.

As we initially insisted  the platform uses gesture capture based on Microsoft’s Kinect as well as a Nuance-powered speech recognition engine. The CEO Adam Odessky consider that this is build for low income people and it seams that Molly speak spanish because she is specialized in the Latin people. We are very glad seeing that our visión is in fact share for other people. And we agree that this lower substantially the cost of Healthcare. Nevertheless, several premises make our proposal different. (1) It is based on intelli-agents capable to evaluate patient data and answer specifically to their requirements. (2) It is based on the Fog and on small data adquisition without interference with the patients. (3) She is not left alone, e-PHAs are directly supervised by PHAs that directly interact with people and patients.

In summary is better integrated in Health 4.0 and IoE.

Nevertheless our vision is not equal. Does anyone ask Why people are living far from IoT? Because they are doomy. No High intelligence is behind because no Deep Learning is implemented with the data obtained. Unfortunatly, in many initiatives machine learning is based  and applied to Big Data.

Our HUMANIZATION OF HEALTHCARE (H2O) paradigm is the following one: For humans, as much data you get less you see or understand. That is the reason why does not make sense to send IoT data to medical doctors. By contrary, Deep Learning machines like a lot of data and Work better with more data available. It also learn from guidelines and evidence based learning based on generalization, but mainly work for individual and specific problems of a patient or a user. This bring a simple way to analyze through the nanopore technology (https://nanoporetech.com/ ) DNA, RNA and even proteins brining the analysis locally in a PC or at distance in the cloud. Have the ability to sequence long reads (i.e. 80,000 base letters long) and up to 150 million bases in six hours. The device cost is $900. The MinION uses a strand sequencing technique for DNA analysis. The DNA strand is attracted to an enzyme that is part of the electrophysiological fluid. In turn, the enzyme is attracted to a nanopore, in which it lodges. The enzyme is engineered to unzip the double-helix structure of the DNA strand so that one strand is directed back into the fluid, while the other enters the nanopore for sequencing
