Do telemedical interventions improve quality of life in patients with COPD? A systematic review Thorbjørn L Gregersen,1 Allan Green,1 Ejvind Frausing,1 Thomas Ringbæk,1,2 Eva Brøndum,1 Charlotte Suppli Ulrik1,2 1Department of Pulmonary Medicine, Hvidovre Hospital, Hvidovre, 2Institute of Clinical Medicine, University of Copenhagen, Copenhagen, Denmark Objective: Telehealth is an approach to disease management, which may hold the potential of improving some of the features associated with COPD, including positive impact on disease progression, and thus possibly limiting further reduction in quality of life (QoL). Our objective was, therefore, to summarize studies addressing the impact of telehealth on QoL in patients with COPD.Design: Systematic review.Methods: A series of systematic searches were carried out using the following databases: PubMed, EMBASE, Cochrane Controlled Trials Register, and Clinical Trials.gov (last updated November 2015). A predefined search algorithm was utilized with the intention to capture all results related to COPD, QoL, and telehealth published since year 2000.Outcome measures: Primary outcome was QoL, assessed by validated measures.Results: Out of the 18 studies fulfilling the criteria for inclusion in this review, three studies found statistically significant improvements in QoL for patients allocated to telemedical interventions. However, all of the other included studies found no statistically significant differences between control and telemedical intervention groups in terms of QoL.Conclusion: Telehealth does not make a strong case for itself when exclusively looking at QoL as an outcome, since statistically significant improvements relative to control groups have been observed only in few of the available studies. Nonetheless, this does not only rule out the possibility that telehealth is superior to standard care with regard to other outcomes but also seems to call for more research, not least in large-scale controlled trials. The aim of this paper is to describe patients’ attitudes towards tele-rehabilitation in the Danish TELEKAT (for Telehomecare, Chronic Patients and the Integrated Healthcare System) project, in order to better understand patients’ behavior when performing tele-rehabilitation activities in home surroundings. A total of 111 COPD patients were included in the study, and they were randomized into an intervention group (n = 60) and a control group (n = 51). However, a non-randomized design was used to analyze the qualitative perspectives of the patients’ attitudes towards tele-rehabilitation. From the intervention group, 22 COPD patients were selected for qualitative interviews and participant observation in their homes. The theoretical framework for this study is based on learning theory and the “communities of practice” approach inspired by Etienne Wenger. COPD patients exhibit four types of attitudes about their tele-rehabilitation: indifference, learning as part of situations in everyday life, feeling of security and motivation for performing physical training. The patients express the view that they circulate between these attitudes depending on their physical and emotional state as they perform their training. The COPD patients and healthcare professionals have created a community of tele-rehabilitation across sectors, exchanging experiences, stories and strategies for how to manage rehabilitation in home surroundings.

 El Instituto de Investigación Médica de Nueva Zelanda (MRINZ) ha anunciado que planea lanzar un estudio clínico multi-país , llamado Novel START, usando los inhaladores inteligentes de Adherium (con sede en Nueva Zelanda). Además de Nueva Zelanda, otros sitios de estudio estarán en Australia, el Reino Unido e Italia.

 


Adherium registra el tiempo y fecha de uso de un inhalador individual, transmitiendo esa información al móvil del paciente y al médico que lo trata. Esto proporciona datos objetivos sobre el uso de los medicamentos de inhalación en los pacientes, médicos y sus cuidadores.

 

Los investigadores planean reclutar a 675 adultos para investigar la seguridad y eficacia de tres diferentes regímenes de tratamiento para el asma leve. El primer régimen de medicamentos utiliza una combinación de corticosteroides inhalados (ICS) y de acción prolongada beta agonista (LABA), según sea necesario. La segunda sólo utiliza agonista beta de acción corta (SABA) según sea necesario. Y el último régimen incluye el uso regular de ICS y Saba, según sea necesario.


"La capacidad de los investigadores para controlar con precisión el uso de la medicación por los pacientes y asegurarse de que los datos del estudio se captura de forma objetiva y precisa es increíblemente útil," dijo el director Richard MRINZ Beasly en un comunicado. "Los datos recogidos en el estudio START Novel serán utilizados como parte del análisis de resultado primaria para determinar el régimen de tratamiento óptimo en el asma leve."
