This patient was referred wearing an implant-supported restoration in the upper jaw associated with a very high mobility of the implants due to periimplantitis. The restoration had only been delivered 10 months earlier and was causing severe discomfort and pain, not allowing the patient to follow the routine oral hygiene instructions. The patient's main wish was to solve the mobility problem of the existing prosthesis, ease the pain during mastication and provide her with a restoration that would allow for a good oral hygiene maintenance and also improve the esthetic outcome.

Interesting video portrays most of the steps involved in dental implantation. From a Vietnamese dental organization. The video includes a very short mention of evidence-based treatment planning at 47 seconds, including cone beam computed tomography, which is the standard of care for most implant cases.

Much of this video is on-target as "informed consent" for patients prior to implant procedures. However, the video makes only a brief mention of evidence-based treatment planning using CBCT or 3D imaging. On a 10-scale, I give this video a 7. Add robust CBCT content and earn a 10.

No doubt CBCT will do the job. However, if the patient comes in three times each year for hygeine - the time when pockets are usually measured - does the industry expose the patient to radiation from CBCT instead?

But after thinking about it, I decided it was a good question, since trying to answer it brings up almost every major topic in the biomechanics of teeth, implants, and bridgework. Here, I’ll try to answer the question for my friend and other interested readers of this issue of JOMI, in case a patient asks anyone the same question.

Mayo Clinic's comprehensive but simple description is very useful. However, it is missing a BIG POINT: treatment planning usually requires a precision X-ray to evaluate bone depth, quality and angulation for implantation. The sole candidate X-ray is the CBCT or cone beam computed tomogram. CBCT is the only techology that enables 3D views through image reformatting.

This is a Nobel Biocare trademarked procedure in which the implants and replacement crowns are fitted all in one go, rather than waiting for six months between the first and second stages. It might suit patients who don’t want to wait for that period of time. However, patient qualifications are exceptionally high due to the need for ample high quality jawbone.

When you walk out of the dentist's office an hour later, your teeth or tooth in an hour is subject to chewing forces that they may not be able to handle. This procedure is expensive and ill-advised for patients with lower jawbone quality and quantity.
