“In recent years, oral antineoplastic agents have transformed the care of cancer patients. They have also brought new challenges, particularly in the case of medication errors.

Oral cancer drugs are “some of the most toxic [compounds] on the market,” and when errors occur, they can be especially dangerous, noted Raymond Muller, MS, RPh, the associate director, Division of Pharmacy Services, Memorial Sloan-Kettering Cancer Center (MSKCC), New York City. Mr. Muller discussed strategies for reducing these errors during a recent webinar sponsored by the Institute for Safe Medication Practices (ISMP).

Chemotherapy mishaps have multiple causes, Mr. Muller noted, including poor packaging/labeling, interruptions during order processing or a misunderstanding of a drug regimen’s total daily dose. A 2010 study identified 508 oral antineoplastic errors through MEDMARX and other databases (Cancer 2010;116:2455-2464). The most common reason for error was wrong dose (38.8%), followed by wrong drug (13.6%), wrong number of days supplied (11.0%) and missed doses (10%). Roughly one-third of the mishaps were due to pharmacy dispensing errors. “That is significantly higher than most medication safety studies,” Mr. Muller said. “Usually, if you look at [the literature], dispensing errors are typically responsible for about 15% of events.”

The app will operate much like a video game. Service members will perform a baseline series of on-screen exercises during which both their speed and accuracy will be recorded. Service members who may have had a serious head injury will then participate in a series of both cognitive efficiency tests and self-administered questionnaires. Afterward, a clinician will review the results, comparing them to the results of the baseline exercises. The combination of the app's cognitive and psychological components allows for insight into the prevalence of symptoms related to both traumatic brain injury and post-traumatic stress disorder.

Functional imaging with SPECT allows for the differential diagnoses of TBI vs PTSD with 100% accuracy. Treatment can now be tailored to the correct condition allowing for better patient outcomes. Considering treatment for TBI and PTSD are different, and pharmaceutical treatment of one condition can be detrimental if treating the wrong diagnosis, this is amazing news!

Last October, I had the pleasure of meeting Mr. Visger at the AANLCP conference in Atlanta, GA; he defies inspiration. As a brain injury survivor myself, we had a chance to speak privately, and laugh together at our shared struggles. Brain injuries NEVER go away; patients never get a "day off". However, some injuries can improve, and some function can return. Yes, March is Brain Awareness Month, however it is a topic that should remain on everyone's mind, every day.
