The outcome is benefits realised, not requirements met (we might have the wrong requirements). So maybe we should start talking about benefits instead of requirements. Might be a better incentive and get us then to work harder at developing proper requirements. All of the British standards talk about benefits realisation. So they work on benefits, not on delivering  deliverables based on bad requirements.
