There is a lot of information, controversy and misunderstanding about this topic, but what is no longer subject to credible debate is that people can engage in patterns of sexual behavior in ways that are capable of bringing immense devastation to themselves and those around them.

 

Phrases such as "sex addiction", "compulsive sexual behavior" and "hypersexuality" each have their advocates and detractors. The curator of this site often uses the generic phrase "problematic sexual behavior" (used as a description rather than a diagnosis) since it:


  • is broad enough to transcend theoretical limitations without being so vague that it loses all meaning,


  • is capable of describing clusters of symptoms and complaints rather than a singular clinical entity,


  • accommodates conditions that stem from varying individual, relational and societal sources, and


  • is therefore a useful inter-disciplinary term to describe the complex psychological, behavioral and relational markers of "patterns of sexual behavior that are out of control and have problematic consequences" (Bancroft and Vukadinovic, 2004*).


Information about this topic will be curated from every conceivable source and perspective, in order not to privilege one viewpoint over another to the detriment of an informed and expanded knowledge base with which to develop a continually evolving understanding of this extremely important subject.  Topics range from rigorous research to irreverent humor, representing the entire spectrum of attitudes, opinions and understanding about this topic.  Some items will have lasting significance while others are likely to merit only brief notice.

Curator comment: This article may seem a bit off-topic, but it helps to continue the process of framing the contours of contemporary society's increasing hypersexualization.  Previous entries have examined such topics as racial stereotypes and  the cultural sexualization of pre-pubescent girls.  This is an entertaining and illuminating look at how televised portrayals of women in their senior years have moved away from depicting them as relatively complex sexual beings to mere "dirty old women".


Follow-up: For a rejoinder expressing the other side of the argument, see  "What's Wrong With the Dirty, Old Betty White?", which makes the following point:


"(T)he rules of what you can do and say on television have become incredibly relaxed, and it only seems appropriate that what’s fine for a man is just as suitable — and funny — for a woman. As long as the Dirty Old Man can be a hilarious stock character, there should be no reason why the same can’t be true for the Dirty Old Woman."

I knew that alcohol and drugs could be addictive, sure. But I’d always seen 'sex addiction' as a lame rationalization for men’s inability to keep it in their pants. Still, watching him desperately trying to explain his behavior, he seemed so powerless and scared. I understood how it felt to need something more, something outside yourself, just to get through a day.

Curator comment: There's no new information here but it's a well-written account of a person's gradual understanding of the similarities between substance addictions like alcohol and process addictions like sex.  Partners who have experienced the gut-wrenching pain caused by the inherent deception that accompanies sex addiction (like all addictions) are generally much more devastated than this writer who had the good sense to move on before forming any deep attachments.

"Understanding the etiology, prevalence, treatment, and associated characteristics of HD is in its infancy. Important questions about the neurobiological mechanisms underlying HD remain unanswered at the present time. Debates continue as to which theoretical conceptualizations of HD might offer the best explanation (e.g., addiction, compulsivity, impulse-control models, etc.). Little is known about the onset, clinical course, and trajectories of HD symptoms as they manifest in various populations and across genders. Public health issues beyond the risks for sexually transmitted infections need to be considered including the impact of hypersexuality on the family, monogamous partnered relationships, child development, and potential risks for suicidal ideation. As advances continue to be made, investigators might benefit from assessing the body of literature about other non-substance related dysfunctional behaviors such as eating disorders or pathological gambling in order to generate hypotheses to guide future research endeavors. Indeed, it is both an exciting and challenging time for researchers and clinicians to be involve."

Curator comment: This is an exceptionally useful article for a number of reasons.  It is authored by one of the most noted and empirically-grounded researchers in the field, Rory Reid Ph.D., LCSW.  It is an extremely helpful and current overview of issues in the field, including descriptions and limitations of various theoretical formulations.  It reviews important issues pertaining to such topics as cultural diversity, comorbidity, brain imaging, public health issues and outcomes research.  It is well annotated. And it's available online in its entirety, not just an abstract.  If you only had a very limited amount of time to get current on this complex issue, make sure this is one of your initial primary sources of information.

"Internet pornography use can be compulsive, but evaluation of pathology underlying this is difficult to assess. The present study aimed to distinguish individual differences in personality and psychopathology that predict pornography consumption in an individual, and whether this reflected more general compulsive processes, assessing 226 male participants. Neuroticism, agreeableness, conscientiousness, and obsessional checking all significantly correlated with a latent measure of compulsive behavior upon which use of Internet pornography use also loaded. The authors suggest the greater use of pornography on the Internet may reflect a general vulnerability to compulsive problems related to basic disposition, and that problems associated with this behavior can be managed with generic clinical approaches to obsessional and compulsive disorders."

"This article focuses on the labor marginalization of black female performers within the pornography industry. Their representations and experiences as sex workers are shaped by a racialized and gendered sexual commerce where stereotypes, structural inequalities, and social biases are the norm. Black women are devalued as hyperaccessible and superdisposable in an industry that simultaneously invests in and ghettoizes fantasies about black sexuality. In light of feminist arguments against the victimization of women by pornography, I have attempted to show that black sex workers, while facing multiple axes of discrimination and harm, also employ hypersexuality and illicit eroticism to achieve mobility, erotic autonomy, and self-care."

Curator comment: It can be painful to witness a person who has committed horrific crimes self-identify as a sex addict if it seems that this is an attempt to slough off responsibility.  The concept of "sex addiction" as it applies to sexual offending is complicated and often emotionally laden.  Robert Weiss has recently written a series of articles about sexual offenders that can be helpful to those seeing more understanding about this complex topic.  I also wrote an editorial in a professional journal a year or two ago about the ethics of carrying the concept of sex addiction too far in the area of nonconsenual sexual behavior.


Addendum (May 13, 2013): Robert Weiss just wrote an excellent piece titled "Decoding Ariel Castro's 'Cold Blooded Sex Addict' Statement" that is well worth reading.

Group Membership and Social Identity in Addiction Recovery

"Although exploratory, these results suggest that developing a social identity as a "recovering addict".......and subsequently highlighting the difference between such identities may be a useful strategy for reducing relapse among people with problems associated with addictive behaviors."

Dissociation and Sexual Addiction/Compulsivity: A Contextual Approach to Conceptualization and Treatment | Sex  Addiction | Scoop.it

The possible relationship of dissociation to sexual addiction/compulsivity (SAC) among childhood sexual abuse (CSA) survivors is explored. In applying a treatment procedure based on functional behavioral analysis to CSA survivors exhibiting SAC it was frequently observed that SAC patterns of behavior were not sexually gratifying, but that clients had been previously unaware of this. These clients often had poor recall of even recent instances of SAC and described a subjective sense of being in a daze, truncated awareness, and absence of a sense of agency during SAC episodes. Repeated detailed description of recent incidents of SAC in session helped clients become less dissociative when subsequently participating in SAC behaviors and gradually reduced the appeal of engaging in them. On the basis of these observations it is argued that recognition of the dissociative quality of SAC behavior in CSA survivors is essential to adequately understanding it, and that targeting the dissociative elements of SAC in treatment is crucial to disrupting it. It is also proposed that in this clinical population SAC can reflect the dissociative disconnectedness represented by re-enactment of CSA experiences, failure to integrate emotional intimacy with sexuality due to insecure or disorganized attachment, association of sexual activity with soothing and validation rather than sexual arousal, or a combination of these factors.

Curator comment: There didn't seem to be a way to summarize this abstract so it is presented in its entirety.  The idea of encouraging people who may be acting out their sexual behavior in a dissociative manner (which is not always the case with the broad population of people who repeatedly violate their sexual commitments in a way that feels out of thier control) to repeatedly recall the specifics of such incidents of "truncated awareness" in order to heighten self-awareness seems sound.

"This study examined the relationship between mindfulness and various addictive behaviors (i.e. alcohol abuse, pathological gambling, sex addiction, and compulsive buying) in a community sample. A short but comprehensive composite measure of mindfulness was constructed...... Inverse correlations emerged between the nonjudgmental attitude and alcohol abuse, pathological gambling, and sexual addiction, ......The findings of this study support the use of mindfulness as a treatment for substance abuse and suggest the possible usefulness for the treatment of other addictive behaviors."

Curator comment: "Mindfulness", a state of nonjudgmental self-awareness, has become a popular topic in the field of behavioral health, and this article continues that trend.  The authors used the Compulsive Sexual Behavior Inventory (CSBI) to measure the sexual dimensions of their study.  It would have been nice if they had shown more consistency by avoiding using the phrases "sex addiction" and "sexual addiction" interchangeably and if they had defined the term other than via the CSBI (which does not use either term).  The "Composite Mindfulness Questionnaire" (which is included) is interesting.


Note: This article is currently available in its entirety through Open Access, which is always nice for those without university or other institutional access.

"Donezepil, an acetylcholine inhibitor, is commonly used in neuropsychiatry for the treatment of cognitive decline and dementia. This report addresses two elderly female patients who had an increase in their libido and experienced hypersexuality after starting donepezil treatment. Potential explanations for this previously unreported association are discussed."

Curator comment: The recent case study about choline and hypersexuality in an elderly client sparked a bit of commentary on a discussion board that included a reference to this case study about donepezil having the same effect. While trying to puzzle this out I came across this study "suggesting that choline and donepezil share a common transport mechanism in the penetration across the blood-brain barrier in vivo." 


Although all of this involves nothing more than speculation of a potential contributor to certain manifestations of hypersexuality in elderly subjects, it seems to be a thread worthy of more tugging.

Curator comment: Even though this is only a case report, it seems reasonable to examine the degree to which dietary supplements may influence hypersexual behavior.  As the abstact concludes: "Since hypersexuality may be an underreported and overlooked adverse effect of drugs and dietary supplements acting on the cholinergic pathway, this should be considered when treating and counselling patients with inappropriate sexual behavior."

Former New York Congressman and current mayoral candidate Anthony Weiner admitted he went to see a therapist in Texas in 2011 at the height of the scandal that lead to his resignation.  He said it was not "rehab" and denied he had been treated for sex addiction. “I didn’t go to rehab anywhere.......A couple of days I worked with a therapist in Texas I was referred to. Two days, twice, for a total of four days. Or, it might have been three.’’

Curator comment: The question that needs to continually be considered is whether some people who consider or describe themselves as sexually addicted more accurately simply enjoy having sex in ways they have difficulty admitting.  The relationship between shame and compulsivity is extremely powerful. This is not a commentary on this particular situation, for no information is provided to document the compulsive, rather than merely repetitive, nature of his behavior.

Curator comment: Based soley on the abstract, this study seems to determine that people who self-identify as "sex addicts" generally expreience distress, often in personal relationships, which seems pretty obvious.  It would be nice to see the entire article.

"Triggers for scrupulosity can be any thought, image, feeling, place, person, etc., that cues an obsession. For example, seeing an attractive person at church may result in sexual thoughts, which in turn trigger an obsessive desire to “undo” that thought in an effort to be pure, holy, and clean. If the scrupulous individual upholds an exaggerated belief that lustful thoughts in and of themselves will automatically result in eternal condemnation, the cycle begins."

Curator comment: Reading this article along with such entries as "The Brain of Someone With OCD is More Active When Faced With a Moral Dilemma" and "The Roles of Shame and Guilt in Hypersexual Behavior" will result in an appreciation for the strong influence of guilt in the cycle of repetitive engagement of unwanted sexual behavior.  This supports the finding in "A Treatment-Oriented Typology of Self-Identified Hypersexuality Referrals" that sexual guilt is a powerful influence on the decision of some people to self-identify as needing treatment for some asepct of hypersexuality.  It can be instructive to also read "Sexual Orientation Obsessions in OCD" to gain even more insight into the complex relationshio between guilt, fear and obsession which can be especially prevalent in certain forms of compulsive sexual behavior.

Curator comment: A recent study revealed that the average age for first viewing online pornography may be as young as 12.  It should be no surprise that some will develop such "obsession" as decribed in the case of this Scottish youth.
