What happened when I open-sourced my brain cancer

What happened when I open-sourced my brain cancer | Patient-Centered Care and Experience | Scoop.it
When artist Salvatore Iaconesi was diagnosed with brain cancer, he refused to be a passive patient -- which, he points out, means "one who waits." So he hacked his brain scans, posted them online, and invited a global community to pitch in on a "cure." This sometimes meant medical advice, and it sometimes meant art, music, emotional support -- from more than half a million people.

How Community Health Workers Can Improve Patient Outcomes

How Community Health Workers Can Improve Patient Outcomes | Patient-Centered Care and Experience | Scoop.it
Spectrum Health, a Michigan health care system serving both urban and nonurban areas, is striving to quantify the benefits that community health workers provide to patients with chronic conditions. The initiative demonstrates how wide-ranging health care networks, like Spectrum, can adapt specific strategies to meet the needs of sometimes very different patient populations. On any given day, community health workers from Spectrum Health United Hospital and Spectrum Health Kelsey Hospital make home visits to residents living near the hospitals’ small-town locations in Greenville and Lakeview, 30 and 45 miles north of Grand Rapids, respectively. They may be following up on a recent hospital discharge, monitoring diabetes or educating patients about how to manage their chronic obstructive pulmonary disease. The visitors' clients appreciate the personal attention and education, and now a Spectrum Health study has documented the visits’ positive impact on health outcomes.

Why Most Clinical Research Is Not Useful

Why Most Clinical Research Is Not Useful | Patient-Centered Care and Experience | Scoop.it
“Useful clinical research” means that it can lead to a favorable change in decision making (when changes in benefits, harms, cost, and any other impact are considered) either by itself or when integrated with other studies and evidence in systematic reviews, meta-analyses, decision analyses, and guidelines. John Ioannidis argues that problem base, context placement, information gain, pragmatism, patient centeredness, value for money, feasibility, and transparency define useful clinical research.

Time for a Patient-Driven Health Information Economy? — New England Journal of Medicine

In 1994, when the World Wide Web was only 2 years old, Massachusetts Institute of Technology computer scientist Peter Szolovits, presaging the consumer health information technology (IT) movement, proposed, in the Guardian Angel Project, using the Web for patient management of health and health data. Yet getting patients electronic copies of their health records has remained an elusive goal. Industry giants have scars to show for their attempts. Why have the barriers been so high? And what is the path to a patient-driven health information economy?

Workforce Activation – Leadership and Empathy

Workforce Activation – Leadership and Empathy | Patient-Centered Care and Experience | Scoop.it

There seems to be ever-increasing focus on empathy in the healthcare space.   We can speculate as to the reasons, but empathy is nothing new. Hippocrates directed Primum No Nocere, “First Do No Harm,” probably while teaching his students under a Planetree. Because healthcare can be intrusive if not invasive, the interviewing, examining and treating of patients without regard to empathy, in my view, can and does create harm. During a healthcare crisis, patients and their families are typically in their most vulnerable states and we find that empathy emerges as a critical component to their care.

Two Sides of a Coin: Being a Patient’s Family & Being the Provider - Planetree Planetalk E-news Article

Two Sides of a Coin: Being a Patient’s Family & Being the Provider - Planetree Planetalk E-news Article | Patient-Centered Care and Experience | Scoop.it

I was working as my hospital’s Manager for Patient Centered Care when I learned the happy news: my wife was pregnant! We were all excited about the news and the endeavor we were about to begin. However, as days passed by we figured out that some things were not normal. My wife had to go through some genetics tests because of abnormal values at the blood tests and ultrasound images. We ended up going to a perinatologist for detailed follow up of the pregnancy. Tests followed other complicated tests and an amniocentesis had to be done.

Randall L. Carter's insight:

A sincere thank you to Ilkay Bylam, Patient Relative & Manager, Patient-Centered Care, Anadolu Medical Center, Turkey for sharing his family's story.

So who are we, patients or consumers? KevinMD

What should we call the people who get medical care? Are they “patients,” the traditional term for anyone receiving medical attention from physicians, nurses, pharmacists, therapists, dentists and other providers? Or are they “consumers,” people who purchase goods and services for personal use?

The debate about nomenclature is growing, and it’s more than a matter of hair-splitting semantics. Among passionate advocates on both sides of the argument, emotions run high.

Co-Production: A New Lens on Patient-Centered Care

Co-Production: A New Lens on Patient-Centered Care | Patient-Centered Care and Experience | Scoop.it
Dr. Paul Batalden, one of the founding fathers of health care quality improvement, was driving down a highway in Minneapolis when he spotted a hospital advertisement that striked him as odd. Whether providers realize it or not, patients are already involved in the creation of health care services. Imagine what good can come if providers put on the lenses of co-production (co-design) to see what more they could do with that reality.
Randall L. Carter's insight:
Welcome commentary and perspective although it is "not a new lens" for those focused on the advanced patient-centered care for decades. As illustrated, there is still great upside potential in our ability to co-design the future of health and care... together. The movement of doing to, to doing for, and most recently doing with, is a progression that those who been championing and executing PCC for years can serve as both example and guides through the sharing of their experience.

Implementing a Care Partner Program: The Epitome of Family Engagement - Planetree Webinar Learning Event | Patient-Centered Care and Experience | Scoop.it

A hallmark Planetree  patient-preferred practice, Care Partner Programs are the epitome of family engagement, leveraging family caregivers as a vital source of continuity, care coordination and communication.  Key program elements formally recognize the experience and expertise that family caregivers bring to the care team, offer training to build their competence and confidence, and provide support for them in this role.  The positive impact of this heightened level of family engagement on patients, families and healthcare delivery system has prompted the adoption of legislation across many U.S. states for hospitals to ramp up efforts to educate and support family caregivers.  This session will provide an overview of the evidence base for Care Partners, identify key implementation steps and sustainability measures, and offer up solutions for common challenges.
