Social Theory and Health - Abstract of article: Fantasy paradigms of health inequalities: Utopian thinking[quest]

The theorization of health issues is crucial both for understanding and as a guide for action. By providing a forum for academics and practitioners to engage with the theoretical development of the health debate, Social Theory and Health aims to develop the theoretical underpinnings of health research and service delivery.

Unfortunately this article is pay-walled but if you can get access it looks to be an excellent read. My suspicion is that most health care students are introduced to the idea of upstream and downstream approaches, but at the end of the day modules (understandably) default to what this article refers to as "the 'cargo cult' of health behaviourism". Ouch!

My view is there is often too much emphasis on self in self management and not enough recognition of the need to provide ongoing support to manage long term conditions. So this is an important reminder of the importance of others.

This article reminds us of the need to remember that all organisations have an agenda and in undertaking a critical evaluation you need to consider what that might be. Its also worth bearing in mind that that bias can shift, for example there have been discussions on twitter about whether the Kings Fund has shifted towards the political right of late.

Handy brief guide to the Care Act and the response of the profession. Although this page provides links to further information that details the concerns of the profession I would prefer it if the 'at a glance' guide' made it clearer that these are the claims of the Government and not the view of the profession. Better care and supporting independent living with a cap on care costs - really? Shifting responsibility from the NHS and onto local authorities makes it easier to make cuts (it was nae us, it is your local authority wot dun it) and introduce charges for services (of course the NHS is still free - we've just handed more of it over to local authorities and if they charge well that's not the NHS).

Given social care services are often lacking, I am yet to be convinced personal health budgets will produce better health services. Health economists have for a long time pointed out that healthcare is an imperfect market yet this initiative seems to further extend the 'marketisation' of health.

This page from the DWP contains information about the closure of the Independent Living Fund (announced March 2014 following judicial review) and transfer of responsibility to local authorities in 2015. This page is likely to have a limited shelf life, expect to have to do a fresh search for developments this time next year (2015).
