Derbyshire firm fined for putting workers in danger

Jessica Mangan's insight:

Confined Space Entry is a fairly common task in my workplace.  We ensure staff are trained and competent, have equipment in good condition and complete a permit.  I struggle to understand that as little as two years ago there were companies who would put employees at risk in situations like this.

Forklift crushes worker - YouTube

Download high quality version here: http://tinyurl.com/5l5yjk Don't rely on just the parking brake. Lower the forks before exiting the cab.
Jessica Mangan's insight:

Forklifts are one of the biggest hazards on any work site.  Whilst forklift operator training and familiarisation is important, training pedestrians and other workers in the same area as forklifts could save lives also.

Inadequate procedures for safely storing flammables

Inadequate procedures for safely storing flammables | Health, Safety, Environment and Training in Manufacturing (OHS QUEST 1) | Scoop.it
Jessica Mangan's insight:

Working on a dangerous goods site, this article was of interest to me to ensure written policies and procedures are available to all employees.  The common-sense argument is interesting to me also, although I have taken from this article the need to train staff in written procedures, rather than just offer instructions and rely on common sense.

Asbestos-related disease - Health & Wellbeing

Asbestos-related disease - Health & Wellbeing | Health, Safety, Environment and Training in Manufacturing (OHS QUEST 1) | Scoop.it
Jessica Mangan's insight:

I currently work on a manufacturing site which is 85 years old, and has asbestos-containg materials throughout.  We have training regimens in place for all employees informing them of the risks, but also informing them of the steps our company has taken to make the workplace safe for them to work in.  We have an ongoing asbestos removal plan in place, and employees are kept informed of this also.  It is important for workers to be informed of the hazards on their site, so they may correctly manage them while carrying out their daily tasks.

▶ Seconds from Disaster S05E05 - The Deepwater Horizon - YouTube - YouTube

Jessica Mangan's insight:

This incident highlights the 'swiss cheese model', where a lot of little things can go wrong at the right time.   Individually these little mistakes would not have such terrible consequences, but when they all line up the results can be catastrophic.  This also highlights the need for logical and effective emergency procedures.  One piece of information that has stuck with me after this video is that the people who followed the emergency procedures were all killed.  The people that disembarked the platform, contradicting the procedures, were the only ones who had a chance at survival.

Hearing loss laws in the workplace | Health and Safety News

Hearing loss laws in the workplace | Health and Safety News | Health, Safety, Environment and Training in Manufacturing (OHS QUEST 1) | Scoop.it
Employers must follow the Control of Noise at Work Regulations 2005 when employees are at risk of damaging their hearing from the work they do. This means employers should eliminate or reduce the risks of hearing problems caused by particular work.
Jessica Mangan's insight:

Hearing loss caused by noise in the workplace is something that may not become evident until later in life.  I feel it is important to look after the health and safety of employees while they are in your workplace, but also take care of them so there are no effects for them later in life.

Fire Company Fined for Scissor Lift Crush Accident - IFSEC Global

Fire Company Fined for Scissor Lift Crush Accident - IFSEC Global | Health, Safety, Environment and Training in Manufacturing (OHS QUEST 1) | Scoop.it
Mobile elevated work platforms (MEWPs), such as scissor lifts, are increasingly used as a safe and versatile means of access to work at height. They’re particularly suitable for fire systems installation and maintenance in larger buildings (with strong floors!). Although they’re expensive to hire or buy, relative to a tower scaffold, there are many advantages …
Jessica Mangan's insight:

This article is a reminder of the importance of equipment training and familiarisation.  My career goal is Health, Safety, Environment and Training Management in Manufacturing, Mining or Resources sectors, where use of mobile equipment is common.
