A natural gas pipeline exploded Tuesday in the Texas Panhandle, killing two construction workers and injuring three others, an official said.

The rapid expansion of the Coal  Seam gas industry in the Surat Basin and surrounding areas raises the possibility of similar incidents occurring in and around our properties. The incident on the CNN clip occurred three miles northeast of Darrouzett, in the Texas Panhandle, killing two construction workers and injuring three others. The gas pipeline was ruptured by mechanical means.  There are however other gas pipe explosions, such as the one that occurred around 7:45 am on Sunday, 14th September 2008 near the town of Appromattox, Virginia. The cause ,outside corrosion of the pipe line which caused the rupture and subsequent explosion. Five people were injured and two homes were destroyed in the blast.

I wonder how much has been done and how prepared we are for similar incidents in Australia.

I live in Dalby and to date have not had any education provided by any gas companies on the hazards, what controls are in place and how we can minimize the risk.

The safety culture of an organization is the litmus test of the safety standards and practices embraced by the the workforce , it is the shared values of the the entire organization. The gap between safety procedures and work place  reality is often a guide to what the actual culture is , not what the OHS practitioner or managers would like to think it is.

 

This article touches on leadership, one of my heros is Jack Welsh. He inspired  leadership . Consistent leadership in safety will drive a positive safety culture. Leaders expect high standards and hold themselves to the same high standards. Good leaders do not tolerate or accept low standards to keep the peace. Good leaders are always consistent and drive home their message with words and deeds.

 

Safety professionals must hold themselves to the highest safety standards and seek to inspire and encourage a positive safety culture in the organization.

 

In 5 years time I would like to be involved in safety leadership training.

Extremely low likelihood events (unpredictable) that would not be considered in any reasonable risk assessment or hazard identification process, yet still do occur. These Black Swan events change our perspectives and redefine what we understand as possible .

Black Swan events are not predicable and therefore fall outside of convention safety thinking. They have no historic relevance or past reference that could be used to create a predictive model.

 

The Black Swan event will test the organizations leadership, the robustness of the organization's ability to react and not it's basic safety management system which could be excellent.

 

Black Swan response teams may become a specialized role in the future to assist organizations in dealing with the event and the follow up investigations and outcomes

I have heard this amazing lady tell her story and raise awareness of the dangers of skin cancer especially melanoma. Queensland has the highest incidence of melanoma cancer in the world, and it is now one of the top two cancer killers of men in our state. From an OHS perspective  the implementation of long pants as mandatory PPE alongside hats and  long sleeve shirts already in place has been frustrating. Despite all the evidence,  consultation and awareness training we still see a huge resistance to positive change. It has taught me that sometimes working in the safety discipline requires that we stand our ground knowing that the advise we have given is based on clear scientific evidence, not what managers, workers or  union representatives necessarily want to hear. It has  taught me to do my home work , know the facts and use the consultative tools at my disposal,but not be swayed by popular myths.
