Medical errors the third leading cause of death?

Medical errors the third leading cause of death? | nursing | Scoop.it
Most medical errors that lead to death aren't recorded. If they were, we'd know for sure how big a health issue they are.
Patti Hamilton's insight:
We are definitely underestimating the risk of weekend care in hospitals.  We use death certificate data in that research.

See what Swedish researchers found when they let a group of nurses work 6-hour shifts instead of 8. | nursing | Scoop.it

Do you live to work, or do you work to live?

Patti Hamilton's insight:

Could this ever be tried here in the US?  Imagine cutting work shifts from 12 to 10 or even 8.  Then think about a 6 hour shift!  Wow.

What do you think might be the result?

Pensioner died after Smethwick care home missed checks - expressandstar.com | nursing | Scoop.it
A pensioner who had difficulty moving and required half-hourly checks was found dead face down in bed after care home staff twice failed to visit her room due to shift pressures.
Patti Hamilton's insight:

I found this story sad but understandable given the interviews we have conducted.  "Rounding" is only helpful when there are enough nurses to do all the other work as well.  What do you believe should be done to prevent such an event in the future?

How the Nursing Work Environment Affects Patient Outcomes - RevCycleIntelligence.com

How the Nursing Work Environment Affects Patient Outcomes - RevCycleIntelligence.com | nursing | Scoop.it
Nurses play a significant role in advancing favorable patient, payment, and nurse outcomes, says a report and accompanying press release from Press Ganey.
Patti Hamilton's insight:

While some of the recommendations seem to be doctrinaire rather than evidence-based there is merit to this report.

I have to wonder, though, how many studies it takes to show that the environment where nurses work affects their attitudes and performance.  Is this not the same with any job or profession?

Reflections of a Nurse: The Little Things Matter - Huffington Post | nursing | Scoop.it
If you hope to be a good nurse (or coworker, or person with a heart), you're going to spend the majority of your working life doing things you SO mistakenly think are beneath you. The "little" stuff is rarely small.
Patti Hamilton's insight:

Since I am interested in "Missed" nursing care I found this blog very interesting.

Could tasks "beneath" a nurse be ones that are missed most often?

This is a bit like trying to apply Maslow's hierarchy to deciding what to complete and what to let slide.

Patti Hamilton's insight:

My heart went out to this nurse and especially to her patients.  Our mental health system is in crisis not only in the US but world wide.

While money would help, compassion, that rare and precious ingredient, must be nurtured and supported.

Health Care Rationing: The Roar and the Silence - Huffington Post

Health Care Rationing: The Roar and the Silence - Huffington Post | nursing | Scoop.it
No discussion means no probing, no deep thinking, no careful exchange of ideas. We do, indeed, ration health care in the United States; we just don't talk about it, with the result that we ration it altogether irrationally.
Patti Hamilton's insight:

Rationing is the feared "R" word in health care.  Of course every time the US passes a national budget, a hospital balances its books, individuals decide what food to buy....they are making rational decisions about resources. That looks like rationing to me.

What are your thoughts?
