Clinical InfoNet - [WEB} resource for chronic disease prevention and management.

The Australian Indigenous ClinicalInfoNet  is a new Internet resource for primary health care workers. It presents, in a single comprehensive resource, existing tools, guidelines and online information to promote best practice in the prevention, identification and management of chronic disease in Aboriginal and Torres Strait Islander people.

It covers the key chronic diseases: cardiovascular disease; diabetes; chronic respiratory disease; kidney disease; and cancer, providing access to: patient education materials; clinical guidelines and references; clinical tools and administrative information covering all stages of the disease pathway.

The integration of the web resource into patient management systems via the PrimaryCare Sidebar. provides access in ‘real time’, enabling immediate returns during the patient consultation.
