The Serratus Anterior muscle group is one of those muscles that when weak  can create havoc to the function of the shoulder. This muscle lies underneath the shoulder blade and attaches to the rib cage at about the level of the pectoralis. When this muscle is weak it limits scapular stability and, therefore, shoulder function. When weak the scapulae will wing. Noted with pressing the against a wall or the floor with the shoulders at 90 flexion. 

This muscle can become weakened by neurological trauma of the long thoracic nerve, or by general postural dysfunction and overuse. Careful assessment should be made prior to engaging in shoulder strengthening exercises.

Strengthening of the Serratus Anterior should be a part of every overhead athlete's shoulder strengthening program. A basic exercise to strengthen the serratus anterior muscle should focusing on endurance, is the push-up plus. This is performed in a hands and knees position, elbows remain straight, and performing a push(separation of the medial angles of the scapulae) then allowing the scapulae to approximate(come together). Perform this repeatedly, with slow movement, for 15-25 reps.

Careful progression should be made to allow the muscles to accomodate to the activity. If the muscle does not respond to the strengthening goals it is strongly recommended that medical evaluation take place to rule out neurological involvement. contact Randy Bauer at www.bauerpt.com if you have questions regarding this topic.
