Voici les prévisions de l'European Travel Commission pour le tourisme émetteur à destination de l'Europe de marchés hors Europe sélectionnés :

 

USA – 84,7 millions de touristes auront voyagé hors des USA en 2013 dont 53,4 (63%) en dehors d'Amérique du Nord. Les arrivées en Europe concentrent 43,8% de ces 53,4 millions, la part principale (38,9% soit 9,1 millions) se rendant en Europe Occidentale (Autriche, Belgique, France, Allemagne, Luxembourg, Pays-Bas, Suisse – Monaco ne communiquant pas encore ces chiffres ne figure pas dans l'enquête). Cependant la part relative de l'Europe Occidentale dans le tourisme US s'est réduite de 1,9 points en 2013 à 17,1% du total. Les arrivées devraient augmenter de 19,5% d'ici 2018 pour atteindre 10,9 millions. La part de marché se réduisant toujours à 15,7%

 

Canada – 35,9 millions de touristes dont 11,3 millions (31,5% en dehors d'Amérique du Nord). La part de l'Europe étant de 40,2%, celle de l'Europe de l'Ouest de 31,6% (1,6 millions) du total de l'Europe. A noter la part très importante de l'Europe pour le tourisme long courrier canadien (14,5% - en baisse cependant de 2,9 points depuis 2003). Malgré une baisse de la part relative de l'Europe Occidentale, le nombre d'arrivées devrait augmenter de 5,2% d'ici 2018.

 

Brésil – 9,47 millions de touristes auront voyagé hors du Brésil en 2013 dont 6,8 en dehors d'Amérique du Sud. Les arrivées en Europe concentrent 51,4% de celles-ci, la part principale (46% soit 1,6 millions) se rendant en Europe Occidentale. La part relative de l'Europe Occidentale dans le tourisme émetteur long courrier brésilien est de 23,6% (en légère baisse –0.6% depuis 2003). Les arrivées en Europe de l'Ouest devraient croître de 16,6% d'ici à 2018.

 

Inde - 602.000 arrivées en Europe Occidentale. D'ici 2018, ce nombre devrait atteindre 1.021.000 en hausse de 69,7%. La part relative de l'Europe Occidentale dans le tourisme émetteur indien devrait progresser de 6% entre 2013 et 2018.

 

Chine – 56,7 millions de touristes dont 33,1 restent en Asie du Nord-Est. L'Europe concentre 33,8% des autres arrivées dont 4,213 millions eu Europe de l'Ouest (17,8% du marché émetteur long-courrier chinois en baisse de 3 points depuis 2003. Le tourisme émetteur chinois devrait croître de 10% par an en moyenne d'ici 2018 pour sa composante long-courrier. La part de marché de l'Europe de l'Ouest devrait rebondir à 19% en 2018 avec 7260000 arrivées (+72,3%)

 

Emirats Arabes Unis – 3,3 millions de touristes dont 2 sont absorbés par le reste du Moyen Orient. L'Europe attire 700000 visiteurs soit 55,9% du marché long courrier. La part de l'Europe de l'Ouest est de 315000. On s'attend à une hausse du marché (+1,8% en moyenne annuelle d'ici 2018) mais à une baisse absolue en Europe de l'Ouest et du Nord et une croissance ne Europe Centrale, Est et Sud.

 

Russie – 41,1 millions de touristes dont 77% restent en Europe. 7,7% en Europe Occidentale, 27,1% en Europe du Sud, 59,6% en Europe Centrale et Est. Les arrivées en Europe Occidentale devraient progresser de 28,3% d'ici 2018 à 3100000 et celles en Europe du Sud de 50,9% à 12900000

When a self-funded employer contracts with a healthcare provider without layering in profit margins of big insurers and traditional PPO networks, they suddenly realize that a few providers in the area are competitive while others directly across the street cost upwards of 240% or more for the same identical service.  If big ticket elective procedures are steered to other markets, local providers will be motivated to bring their rates into a reasonable price range or face loss of market share. 


In severe cases where the value doesn’t exist because providers don’t feel pressure to lower local prices, employers might only encourage the use of local market providers for emergencies and urgently needed care, or when the procedure or treatment is required by a patient that cannot safely travel to a nearby or distant medical hub of health tourism cluster offering price or value arbitrage.

When one considers certain key structural and cultural elements of the medical profession – a powerful place at the top of the professional hierarchy; a fierce attachment to individual professional autonomy; a deep-seated wariness of corporate structures; and lack of alignment of business interests with both other physicians, and with hospitals and other institutions – it is not surprising that whether in medical tourism or public health, the task of improving and assuring clinical quality of care depends largely on the ability to organize physicians into some sort of integrated group setting.


At the same time, hospitals and their physicians are increasingly in competition with each other. In medical tourism, many hospitals have begun to employ various types of physicians whom independent medical staff members fear will steal their business, while the independent physicians themselves build and develop rival outpatient surgicenters, imaging facilities, and even full-service hospitals.  Not only in the United States but also in other countries, organized medicine and healthcare cluster entities often have trouble generating much enthusiasm from physicians  who are challenged by the demands of their daily professional and business lives, including onerous administrative and regulatory compliance burdens, inadequate compensation, escalating malpractice premium expense, increasing staffing costs, and overall decreased satisfaction with their roles as physicians.

So what are the best ways to engage them to integrate and align in a cluster setting for health tourism?
