leaders and their organizations suffer from fear of failure and practiced incumbency, especially when embarking on an ambitious change initiative.
 
The change journey is fraught with unknowns and the unexpected. Orchestrating organizational transformation in any large company is like launching a rocket into space. Much goes into preparations to be successful, but unforeseen factors like weather or wind pressure can affect the launch and flight path. A multitude of factors can misdirect the change program, resulting in delays, crashes, or becoming lost in orbit.

If you are a leader of an organization, unit or team, ensure that this year you carve out some time regularly to reflect on these kinds of strategic questions.  It can be monthly or 6-monthly but put the time slot in you diary now and protect it before the the time gets eaten up and you become too 'busy' again.

Around the world, participation rates in tertiary education are on the rise and one of the key challenges facing educators is finding ways to engage these students. We present the results of a project that assesses the impact of an engagement strategy in which a cohort of students entering their first year of university (1) establish and maintain a clear goal of their ideal future career and (2) make use of a web-based digital curation tool to research and present their findings.

I completely agree and think that we need to take on board what Prof Hollnagel is saying. Look at how the systems really work successfully i.e work as done and not work as imagined (by the designer), Use the best of what humans do well and stop blaming failures on human error.
