Poor communication between the physician and patient or any of the care team memebers crerates a challenging work environment and puts the patient's safety at risk. Can't we all just get along? Humanizing each other and building trusting relationships through respectful communication is key.

Many do not see such incident as work related deaths, yet hospitals are workplaces and must be held accountable like any other workplace and those at the top including politicians must accept responcibility for their failings.  Those on high salaries from the politician to the arms length qangos (including private companies) that politician try to outsource their responcibilities to, must personaly be made to pay for their failings.
