My own two cents is that it is a combination of the three. Mostly it is people not understanding who they should recruit in the first place. By making a recruitment profile simply on hard values, the most important part is forgotten; who is actually most able to become loyal longterm and achieve?

What I always stated myself repeatedly to everyone. It is about your culture, your personality and what personifies you. You being authentic. It is not about trying to score high in 3-4 parameters from what quantitative research state is important. Then you'll never be more than that and also, one among many who go for the same thing. Your culture however is unique and attractive to those who matters, if rightly made. Build and highlight your culture and be successful.
