American hospitals have a big problem with unnecessary deaths from medical errors. Estimates of the numbers vary widely, but extrapolating from the best studies, a conservative estimate would be that well over 100,000 people a year die unnecessarily because of errors made by their healthcare teams. And the numbers have remained high despite concerted efforts to bring them down. Why? Because we've embraced a so-called solution that doesn't address the problem.

Background Under the Family Smoking Prevention and Tobacco Control Act (FSPTCA), the Food and Drug Administration (FDA) banned the use of “Lights” descriptors or similar terms on tobacco products that convey messages of reduced risk. Manufacturers eliminated terms explicitly stated and substituted colour name descriptors corresponding to the banned terms. This paper examines whether the tobacco industry complied with or circumvented the law and potential FDA regulatory actions.

Au-delà de ce constat, le Livre blanc propose quelques bonnes pratiques ainsi que six mesures à prendre d’urgence. Objectif : sensibiliser les pouvoirs publics et les professionnels de santé.Edité sous l’égide de la Fondation Concorde et sous la direction du Dr en psychologie clinique Denis Fompeyrine, l'ouvrage assène des chiffres qui font peur : " environ 50 % des hypocholestérolémiants, des antihypertenseurs ou des antidiabétiques oraux prescrits ne sont pas pris".En France, la non observance concerne donc près de la moitié des patients atteints de maladies chroniques dont le nombre ne cesse d'augmenter, et l'on estime son coût à 2 milliards d'euros par an, les journées d'hospitalisation induites à 1 000 000 et les décès à 8 000 (2).Il s'agit de la forme de mésusage la plus dangereuse et la plus répandue : des personnes âgées qui se trompent de posologie, des patients greffés qui ne prennent pas leurs anti-rejets ou encore des patients diabétiques qui oublient leurs traitements.

Respecter , observer ... nous sommes dans le registre de l'obéissance . Or nous avons affaire ici à des personnes qui devront jour après jour vivre avec une  maladie : nous , les bien portants , pouvons-nous affirmer que tous les jours de toute notre vie nous serions "parfaits" ? Quant aux solutions des rappels ( SMS ... ) , ce peut être un  pas de plus dans la dépendance , voire l'infantilisation , alors que le but à atteindre est que notre patient 1) puisse décider en connaissance de cause du niveau de risque qu'il peut de lui-même  fixer et 2) qu'il puisse nous en parler sans avoir le sentiment d'être au confessionnal ...

The 2007 Food and Drug Administration (FDA) Amendments Act expanded requirements for ClinicalTrials.gov, a public clinical trial registry maintained by the National Library of Medicine, mandating results reporting within 12 months of trial completion for all FDA-regulated medical products. Reporting of mandatory trial registration information on ClinicalTrials.gov is fairly complete, although there are concerns about its specificity; optional trial registration information is less complete.14 To our knowledge, no studies have examined reporting and accuracy of trial results information. Accordingly, we compared trial information and results reported on ClinicalTrials.gov with corresponding peer-reviewed publications.
