Does the Gut Microbiota Trigger Hashimoto’s Thyroiditis? - Kouki Mori - Discovery Medicine | diabetes and more | Scoop.it

Hashimoto's thyroiditis is an organ-specific autoimmune disease in which both genetic predisposition and environmental factors serve as the trigger of the disease. A growing body of evidence suggests involvement of viral infection in the development of Hashimoto's thyroiditis. However, not only pathogenic microorganisms but also non-pathogenic commensal microorganisms induce proinflammatory or regulatory immune responses within the host. In accordance, series of studies indicate a critical role of intestinal commensal microbiota in the development of autoimmune diseases including inflammatory bowel diseases, type 1 diabetes, rheumatoid arthritis, and multiple sclerosis. In contrast, the role of the gut and indigenous microorganisms in Hashimoto's thyroiditis has received little attention. Whereas activation of innate pattern recognition receptors such as Toll-like receptors and disturbed intestinal epithelial barrier may contribute to thyroiditis development, only a few studies have addressed a link between the gut and Hashimoto's thyroiditis and provided just indirect and weak evidence for such a link. Despite this unsatisfactory situation, we here focus on the possible interaction between the gut and thyroid autoimmunity. Further studies are clearly needed to test the hypothesis that the gut commensal microflora represents an important environmental factor triggering Hashimoto's thyroiditis.

The other day, my dad told me that he’d recently hired a woman at his company who has LADA (Latent Autoimmune Diabetes in Adults). She wears an insulin pump, and when he mentioned that I went off my pump this Spring and went back to taking multiple shots a day, the woman was apparently shocked and couldn’t figure out why I would “go backwards.”

It’s true that switching from a pump to multiple daily injections (MDI) isn’t nearly as common as people who switch from shots to a pump. But people do it. And I’m one of them. It’s not that I don’t like insulin pumps or that they don’t work. It’s just that when my pump happened to break back in March, I decided to take advantage of the opportunity to have a “pump vacation.”

I’ve discovered that one of the most difficult parts of being off the pump is doing the D-math that my pump used to do for me......

The UK pharma industry association told PMLiVE that biosimilars should undergo “the standard full health technology assessment processes” so that they can be “appropriately assessed for clinical and cost effectiveness”.
Unlike generic versions of small molecule drugs biosimilars are based on similarity to an original product, rather than bioequivilance, a difference the ABPI is concerned will be overlooked.
The association recently released a position paper covering seven areas where it says action on biosimiliars is needed by regulators, HTA agencies, NHS commissioners and NHS healthcare professionals.
These include clinical testing, something the ABPI says should be greater than at present and it calls for all biosimilars to “demonstrate safety and efficacy through robust phase III trials in exactly the same manner as the originator medicine”....
