TMJ Health (Farmington Hills, MI) has received FDA clearance for the TMJ NextGeneration Device to alleviate temporomandibular joint disorder (TMJD) pain. TMJD affects the jaw joint, surrounding muscles, and nerves, leading to chronic pain and poor quality of life. According to the NIH, more than 35 million Americans suffer from TMJD, which includes symptoms such as pain in the face, jaw or neck, jaw muscle stiffness, and sharp pain around the ears. TMJD has conventionally been treated with bite splints to reduce the amount of grinding of the teeth that causes the sharp pain. These splints cannot be worn while eating, are known to affect speech, and cause long term damage to the joints.

The TMJ NextGeneration device treats the TM joint pain in the ear canal, which happens to be near the temporomandibular joint. The device consists of two hollow ear canal inserts that rest in the outer part of the ear canal and allow full transmission of sound due to their hollow construct. The device is also easy to remove thanks to the retraction posts that are built into it.  The device is discreet, not noticeable from the outside, and makes use of the dynamic change in the ear canal geometry when the jaw is opened or closed through movements such as chewing and smiling. The device is 20% larger in size compared to the ear canal when jaws are closed, and this change in ear canal space applies light pressure against the walls of the ear canal to encourage the patient to return to the open jaw position while reducing the inclination to clench the jaw. All TMJ Next Generation devices are custom built to match the patient’s unique anatomy.

The available data suggest that treatment with an air-powder abrasive system with sodium bicarbonate powder does not seem to adversely affect the biocompatibility of titanium dental implant surfaces. However, the clinical impact of these findings requires further clarification.

The aim was to assess the role of platform switching (PS) in minimising crestal bone loss around dental implants through a systematic review of the currently available clinical evidence. To address the focused question ‘Does PS minimise crestal bone loss compared with non-platform-switched (NPS) implants?’, PubMed/Medline and Google Scholar databases were explored from 1986 up to and including December 2013 using the following key words in different combinations: ‘bone loss’, ‘dental implant’, ‘diameter’, ‘mandible’, ‘maxilla’ and ‘platform switching’. Letters to the Editor, unpublished data, historical reviews, case reports and articles published in languages other than English were excluded. Fifteen clinical studies were included. In seven studies, PS and NPS implants were placed in both the maxilla and mandible. In 13 studies, implants were placed at crestal bone levels whereas in one study, implants were placed supracrestally. Three studies reported the bucco-lingual (or transversal) width of the alveolar ridge which ranged between 7–8 mm. Seven studies reported that implants placed according to the PS concept did not minimise crestal bone loss as compared with NPS implants. 3D-Implant positioning, width of alveolar ridge and control of micromotion at the implant-abutment interface are the more critical factors that influence crestal bone levels than PS.

The single-drill method induced a significantly greater variation in temperature than the traditional method, but only when irrigation was used; without any irrigation, the difference in the temperature variation generated by the two methods was not statistically significant. In any case, bone heating during the osteotomy never exceeded 2°C and was clinically irrelevant, as thermal damage to bone has only been reported in the literature for temperatures beyond 47°C persisting for more than one minute.

To assess gingival recession (GR) in manual and power toothbrush users and evaluate the relationship between GR and gingival abrasion scores (GA). 

Results

Overall, 97.8% of the study population had at least one site of ≥1 mm of gingival recession. For the manual group, this percentage was 98.9%, and for the power group, this percentage was 96.7% (P = 0.621). Post-brushing, the power group showed a significantly smaller GA increase than the manual group (P = 0.004); however, there was no significant correlation between number of recession sites and number of abrasions for either group (P ≥ 0.327).
