In Portland, Maine a group called Amadeus support people with mental health diagnosis who are stable to coach others with mental health diagnosis who need to be.  They understand the process and are excellent coaches. The guy I interviewed laughed, "I read this Craigslist add and thought Crazy people helping crazy people - this has got to be a joke!" It's no joke.  I saw it in action and it works!

Population management means the hospital is responsible for its surrounding geographical area's health - everybody not just patients.  It is a groundbreaking innovation that creates a key mechanism (Jim Collins term) that flips the goals of healthcare providers away from old profit centers that thrived on sick people.

"The state isn't doing it," Hope-Hernandez said. "The governor and insurance commissioner have been openly hostile to the Affordable Care Act. The attorney general is still trying to sue the federal government over it.


In states openly hostile to the Affordable Care Act...someone else (probably insurance companies who want more busines) will have to publicize and educate citizens of their affordable insurance opportunties.  The Fed doesn't really have the PR budget for states that opt out.

Every article that exposes over charging should help consumers of healthcare...and yet, the group purchasing organizations that keep prices high (we think, the Dept. of Health wont release the information) may be immune to consumer protection efforts like this disclosure.

 

I just got an alert that yet another "group purchasing organization" Premier is issuing an IPO. Who knew they were such big players?  Hospitals are supposedly using them to negotiate better prices, but these massive organizations are in the right place to exploit their position. Apparently they are eating up the competition and getting bigger and bigger.

The Obama administration has put off another provision — on deductibles and co-payments — until 2015.
Annette Simmons's insight:

The delay for a cap on out of pocket expenses is disappointing.  The complexities of redesigning electronic records in a way that makes sense for the thousands of diseases, diagnoses, and injuries as well as is usable by doctors who are already overwhelmed with journal articles they havent read PLUS satisfying the insurance companies and goverment payers....well no wonder there will be a delay.

 

They aimed high, becasue that's what Americans do, we aim high.

Best of all everyone hears the same story.

Using games to educate people on their health insurance.  I wish they had educated me on mine.  I just discovered that Blue Cross Blue Shield authorized an MRI for me with a provider that was in network a month ago and is now out of network.  Why get an authorization if it doesn't mean I have a good chance of getting reimbursed?  We all know an authorization is "not a guarantee of reimbursement" but c'mon...

 

Anyway, I like that this group is trying to educate their members on what is and is not included in their health insurance.  Who can possibly wade through all the documents they send?

Ohio groups trying to publicize the exchanges have had a particularly difficult time because a new state law prohibits anyone not paid by the federal government to say they are navigators, the representatives of groups authorized to help people enroll in insurance.


State sponsored obstruction to the success of the Affordable Health Care law.  Shameful.

The patient centered medical home is as much an educational program as it is a "healthcare provider."  It is time to harvest all the things we know about education and behavioral science and stop using theory from the medical communities.  I've found the behavioral interventions being used simply to improve communication and spread innovations have a lot to be desired.  The way healthcare does research is so focused on isolating factors that they learn about humansvery slowly. None of our factors are really isolated and the variability of emotions isn't well captured in this kind of research.

In five weeks from now, the Patient Protection and Affordable Care Act (ACA) mandates the opening of health insurance exchanges around the country. At that time New Yorkers will be introduced to an innovative way of thinking about health care: Oscar.

When you have all the data you need in your phone app, you can make better decisions for yourself.  Local to the point of personal.  A good idea as long as the data is used for good! What if they find another way to marginalize "expensive" patients who suffer from chronic diseases?
