•Naloxone is a specific antidote to opiate overdose, it reverses central depression, including respiratory depression, induced from either natural or synthetic opioids. It is used when coma and bradypnoea (slow breathing) are present. •Naloxone competes with the opiates for the opiate receptor sites. •Naloxone has a rapid onset of action, usually within 2 minutes. •Naloxone has a shorter duration of action than most opiates, the effect lasts between 20 minutes to an hour, depending on the route of administration and the dose, therefore close monitoring and repeat injections (at 2-3 minute intervals) are necessary depending on respiratory rate and depth of coma. For opiates with long duration of action (e.g. methadone), naloxone may need to be given by iv infusion over a period of time, with longer term monitoring. •Some opiates such as buprenorphine and fentanyl are only partially reversed due to their high affinity for the receptor sites.
