Teen dies after workplace accident

Teen dies after workplace accident | Accident Forensics and OHS | Scoop.it
An 18 year-old man who was caught in a printing machine at a carton-manufacturing plant in Thomastown yesterday has died.
Jess Kopp's insight:

The age limit of death in not set, anyone can be the victim of a work place related death. This death was caused by the young males clothing becoming caught in the machine he was working on that dragged him to his death, any one of a number of things could have prevented this death, more fitted clothes, more supervision, more instructions. Everything can be improved, anything can make the slightest of difference.

Jess Kopp's insight:

Although it is not heard about often enough, there are many risks involved in a police officer going to work and working more than one shifts. As the shifts get longer their awareness can drop this can cause more accidents while they work, these accidents range from first aid status to life threatening.  Though they do not often occur there have been accidents involving squad cars, these are caused by travelling at high speeds, fatigue, switching from high beam to low beam constantly. The rate of fatalities also seems to be higher with these crashes, and this is said to be the fault of the higher speeds involved.

Faults found in Cootes fleet, 50 defect notices issued to trucking firm

Faults found in Cootes fleet, 50 defect notices issued to trucking firm | Accident Forensics and OHS | Scoop.it
THE trucking company at the centre of Tuesday's deadly crash in Mona Vale has been issued 50 defect notices after a thorough inspection of its fleet.
Jess Kopp's insight:

This proves that there are people in the world who would risk the lives of their employees and citizens of their community in order to cut corners. Over 50 defect notices at once and causation of a fatal accident can be worth a lot more money in the fines, costs to repair the trucks. But no amount of money is worth someones life.

Drivers warned as chill brings deadly black ice to roads

Drivers warned as chill brings deadly black ice to roads | Accident Forensics and OHS | Scoop.it
It has been a cold winter’s morning in Melbourne, and a positively chilly one in Ballarat.
Jess Kopp's insight:

This is an excellent example about how it is not always an accident caused by humans or the failure of technology but how sometimes it can be nature that is to blame for fatal accidents, and how when travelling motorists need to take the weather into consideration and adjust their speeds to the conditions.

Video: Pa. cop risks life to save endangered officer, awarded at IACP | Accident Forensics and OHS | Scoop.it
By PoliceOne Staff FRANKSTOWN TOWNSHIP, Pa. — Trooper Timothy Strohmeyer was awarded the International Police Officer of the Year Award at the 2013 IACP conference, for his heroic efforts to save another officer — an incident that was caught on dash cam. Jeffrey Lee
Jess Kopp's insight:

Even though its not based in Australia i still think that this story is amazing, one police officer risked his own life in order to save the life of one of his colleagues, everyone knows that being a police officer is a dangerous job with many opportunities for work related deaths and accidents. This officer not only saved his colleagues life, he prevented another work place related death and saved one family of heartbreaking news, he deserved all of the awards given and more for risking his own life to save another.

Notifiable%20Fatalities_November2013.pdf

Jess Kopp's insight:

The above information contains statistics about work related fatalities, in 2013 there were 388 deaths that were related to work. There are many laws, rules and legislations in place to avoid this amount of deaths, so why dont the employers and employees follow them? It is for their safety and for the innocent bystanders safety as well.

2013_NTI_Major_Accident_Investigation_Report_-_Web.pdf

Jess Kopp's insight:

Reading through this report there is a lot of surprising and some not so surprisings facts and statistics. For the majority of the roads the amount of vehicles are increasing so are the amount of crashes. However the trucking industry must be doing something right as the amount of fatal accidents involving trucks have been lesser of the truck drivers fault but more of the third part or the smaller vehicles fault.

Company fined after work accident leaves man a quadriplegic (1)

Company fined after work accident leaves man a quadriplegic (1) | Accident Forensics and OHS | Scoop.it
Broken Hill mining company Perilya has been fined $260,000 after an accident left a worker quadriplegic.
Jess Kopp's insight:

An excellent example on how people or in this case companies will never learn, not only have they be fined three separate times due to not having the right procedures and equipment in place in order to prevent accidents but there have been multiple people affected due to their lack of responsibility.

Four in every five crashes between cars and bicycles caused by driver of car

Four in every five crashes between cars and bicycles caused by driver of car | Accident Forensics and OHS | Scoop.it

rFOUR in every five crashes between cars and bicycles are caused by the driver of the car, police statistics show.

Jess Kopp's insight:

With the amount of motorists constantly increasing there is more risk becoming involved in travelling on the roads. With the increase of motorists also comes with an increase of responsibility, when you are driving on the roads you need to be more aware of your surroundings, the danger is not only in front of you it can be to the side or behind your vehicle and the danger is not only for yourself it can be towards those that surround you. People need to learn that it is not only themselves that the need to drive safely and alertly for it is for the community and the citizen within it.
